浸润性乳腺癌组织中Her-2、ER、PR、Ki-67的表达及临床意义 被引量:2

摘要 目的:探讨浸润性乳腺癌组织中人表皮生长因子受体-2(Her-2)、雌激素受体(ER)、孕激素受体(PR)、细胞增殖指数(Ki-67)的表达水平,并分析其与肿瘤T、N分期的相关性。方法:回顾性分析2020年1月至2021年1月该院诊治的71例浸润性乳腺癌患者的临床资料。在超声引导下对患者行穿刺活检,取乳腺癌组织及癌旁组织行免疫组化检测,比较乳腺癌组织与癌旁组织,以及不同T、N分期乳腺癌病灶中Her-2、ER、PR、Ki-67的阳性率。采用Spearman相关性分析Her-2、ER、PR、Ki-67阳性率与T、N分期的相关性。结果:乳腺癌组织中Her-2、Ki-67阳性率明显高于癌旁组织,ER、PR阳性率明显低于癌旁组织,差异均有统计学意义(P<0.05);Her-2、Ki-67阳性率比较,T_(1)期<T_(2)期<T_(3)期<T_(4)期,N_(1)期<N_(2)期<N_(3)期;ER、PR阳性率比较,T_(1)期>T_(2)期>T_(3)期>T_(4)期,N_(1)期>N_(2)期>N_(3)期,差异均有统计学意义(P<0.05);Her-2、Ki-67阳性率与T、N分期均呈正相关(r>0,P<0.05),ER、PR阳性率与T、N分期均呈负相关(r<0,P<0.05)。结论:Her-2、ER、PR、Ki-67可作为肿瘤标志物检测浸润性乳腺癌,其中Her-2、Ki-67阳性率与T、N分期均呈正相关,ER、PR阳性率与T、N分期均呈负相关。 Objective:To investigate expression levels of human epidermal growth factor receptor-2(Her-2),estrogen receptor(ER),progesterone receptor(PR)and cell proliferation index(Ki-67)in invasive breast cancer tissues and to analyze their correlations with tumor T and N staging.Methods:The clinical data of 71 patients with invasive breast cancer diagnosed and treated in this hospital from January 2020 to January 2021 were retrospectively analyzed.Ultrasound-guided biopsy was performed,and the breast cancer and the adjacent tissues were taken for immunohistochemical detection.The positive rates of Her-2,ER,PR and Ki-67 in the breast cancer lesions and the adjacent tissues as well as the breast cancer lesions of different T and N stages were compared.Spearman correlation analysis was used to explore the correlations among the positive rates of Her-2,ER,PR,Ki-67 and the T and N stages.Results:The positive rates of Her-2 and Ki-67 in the breast cancer tissues were significantly higher than those in the adjacent tissues,the positive rates of ER and PR were significantly lower than those in the adjacent tissues,and the differences were statistically significant(P<0.05).The comparison of the Her-2 and Ki-67 positive rates showed that T_(1)<T_(2)<T_(3)<T_(4),N_(1)<N_(2)<N_(3);the comparison of the positive rates of ER and PR showed that T_(1)>T_(2)>T_(3)>T_(4),N_(1)>N_(2)>N_(3);and the differences were statistically significant(P<0.05).The positive rates of Her-2 and Ki-67 were positively correlated with the T and N stages(r>0,P<0.05),while the positive rates of ER and PR were negatively correlated with the T and N stages(r<0,P<0.05).Conclusions:Her-2,ER,PR and Ki-67 can be used as the tumor markers to detect invasive breast cancer.The positive rates of Her-2 and Ki-67 are positively correlated with the T and N stages,while the positive rates of ER and PR are negatively correlated with the T and N stages.
