前列腺癌患者生存质量的影响因素分析

Analysis of Influencing Factors of Quality of Life in Patients with Prostate Cancer

目的 调查分析影响前列腺癌患者生存质量的因素,为前列腺癌患者生存质量的全面管理提供依据。方法 采用方便抽样法,选取2020年3—9月于四川大学华西医院泌尿外科拟接受根治性前列腺癌切除术的108例患者为研究对象。通过一般资料调查表、体能状况评估表(ECOG)、国际前列腺症状评分表(IPSS)、国际勃起功能指数问卷表-5(IIEF-5)、Herth希望指数量表(HHI)和生存质量测定量表简表(QOL-BREF)进行问卷调查。结果 年龄与生存质量Pearson相关性分析为负相关,即随着年龄增大生存质量总分下降(r=-0.239,P<0.05);勃起功能、对现实和未来的积极态度、采取积极的行动、与他人保持亲密关系与生存质量的Pearson相关性分析为正相关,即其得分越高生活质量越好(r=0.212、r=0.211、r=0.419、r=0.386,P<0.05);将患者年龄、勃起功能、对现实和未来的积极态度、采取积极的行动、与他人保持亲密关系作为自变量,生存质量作为因变量进行多因素线性回归分析,结果显示,采取积极的行动与生存质量差异有统计学意义(t=2.838,P<0.05),并可建立方程:生存质量=91.754+2.714×采取积极的行动。结论 年龄、勃起功能、对现实和未来的积极态度、采取积极的行动、与他人保持亲密关系是前列腺癌患者生存质量的影响因素。对于前列腺癌患者,不仅要重视疾病症状及功能障碍的治疗与管理,更应采取积极的诊疗和护理策略提高其生存质量。

Objective To investigate and analyze the influencing factors of the quality of life of patients with prostate cancer,so as to provide reference for the comprehensive management of their quality of life.Methods A total of 108 patients with prostate cancer who were scheduled to undergo radical prostatectomy in the urology department of West China Hospital of Sichuan University from March to September 2020 were selected as the study objects by convenient sampling.General information questionnaire,eastern cooperative oncology group(ECOG),international prostate symptom score(IPSS),international index of erectile function-5(IIEF-5),Herth hope index(HHI),and quality of life brief(QOLBREF)were used for the questionnaire survey.Results Pearson correlation analysis revealed a negative relationship between age and quality of life,indicating that the total score of quality of life decreased with age(r=-0.239,P<0.05).And Pearson correlation analysis of erectile function,positive attitude towards reality and future,taking positive actions,maintaining close relationships with others and quality of life revealed a positive correlation,indicating that the higher the score,the better the quality of life(r=0.212,r=0.211,r=0.419,r=0.386,P<0.05).Taking patients'age,erectile function,positive attitude towards reality and future,taking positive actions and maintaining close relationships with others as the independent variables,and quality of life as the dependent variable,the multivariate linear regression analysis revealed a statistically significant relationship between positive actions and quality of life(t=2.838,P<0.05).As a result,the equation could be established:quality of life=91.754+2.714×taking positive actions.Conclusion Age,erectile function,positive attitude towards reality and future,taking positive actions,and maintaining close relationships with others are factors that influence the quality of life of prostate cancer patients.Thus,for prostate cancer patients,not only attention should be paid to the treatment and management of disease symptoms and dysfunction,but also active diagnosis,treatment and nursing strategies should be adopted to improve their quality of life.
