异甘草酸镁治疗药物性肝炎患者的疗效与安全性的meta分析

摘要 目的:系统评价异甘草酸镁对药物性肝炎患者治疗的有效性和安全性。方法:系统检索PubMed/MEDLINE、Cochrane Library、Embase、Web of Science、Scopus、CNKI(中国知网)、WANFANG DATA(万方数据)数据等电子数据库中还原型谷胱甘肽或甘草酸单铵与异甘草酸镁在药物性肝炎患者中应用效果比较的随机对照试验相关文献,检索时限为1966年—2023年4月。采用Rev-Man 5.0对文献结果进行meta分析。结果:异甘草酸镁的治疗总有效率显著高于还原型谷胱甘肽或甘草酸单铵(OR=4.26,95%CI=2.80~6.46,P<0.001);异甘草酸镁对肝功能指标的改善效果优于还原型谷胱甘肽或甘草酸单铵(MD=-21.06,95%CI=-25.28~-16.85,P<0.001);还原型谷胱甘肽或甘草酸单铵与异甘草酸镁治疗的不良反应率无统计学差异(OR=0.37,95%CI=0.13~1.05,P=0.55)。结论:与还原型谷胱甘肽或甘草酸单铵治疗比较,采用异甘草酸镁治疗药物性肝炎的临床效果更佳,不良反应较少,值得临床应用。 Objective To systematically evaluate the efficacy and safety of magnesium isoglycyrrhizinate in the treatment of drug-induced hepatitis.Methods The related literatures of Randomized controlled trial in the electronic databases such as PubMed/MEDLINE,Cochrane Library,Embase,Web of Science,Scopus,CNKI(China Knowledge Network),WANFANG DATA,etc.,which compare the application effect of reduced glutathione or monoammonium glycyrrhizinate and magnesium isoglycyrrhizinate in patients with drug-induced hepatitis,were systematically searched.The search period from 1966 to April 2023.Using Rev-Man 5.0 for meta-analysis of literature results.Results The total effective rate of magnesium isoglycyrrhizinate was significantly higher than that of reduced glutathione or monoammonium glycyrrhizinate(OR=4.26,95%CI=2.80~6.46,P<0.001);The improvement effect of magnesium isoglycyrrhizinate on liver function indexes was better than that of reduced glutathione or monoammonium Glycyrrhizin(MD=-21.06,95%CI=-25.28~-16.85,P<0.001);There was no difference in the rate of adverse reactions between reduced glutathione or monoammonium glycyrrhizinate and magnesium isoglycyrrhizinate(OR=0.37,95%CI=0.13~1.05,P=0.55).Conclusion Compared with the intervention treatment of reduced glutathione or monoammonium glycyrrhizinate,the clinical effect of magnesium isoglycyrrhizinate in the treatment of drug-induced hepatitis is better,with less adverse reactions,which is worthy of clinical application.
