肺磨玻璃结节HRCT征象对微浸润性腺癌与浸润性腺癌的鉴别诊断价值

Value of HRCT Signs of Pulmonary Ground Glass Nodules in the Differential Diagnosis of Microinvasive Adenocarcinoma and Invasive Adenocarcinoma

目的分析肺磨玻璃结节高分辨率CT(HRCT)征象对微浸润性腺癌(MIA)与浸润性腺癌(IAC)的鉴别诊断价值。方法62例行胸部HRCT检查的肺磨玻璃结节腺癌患者根据术后病理诊断结果分为MIA组和IAC组,比较两组的肺磨玻璃结节HRCT影像学特征表现。结果两组的结节类型、结节形状、空气支气管征、血管集束征、胸膜凹陷征、结节大小及平均CT值比较,差异有统计学意义(P<0.05)。结论MIA与IAC患者的肺磨玻璃结节HRCT征象存在差异,其中肺磨玻璃结节类型、形状、空气支气管征、血管集束征、胸膜凹陷征、结节大小及平均CT值对临床鉴别MIA和IAC具有重要的指导意义。

Objective To explore the value of high resolution CT(HRCT)signs of pulmonary ground glass nodules in the differential diagnosis of microinvasive adenocarcinoma(MIA)and invasive adenocarcinoma(IAC).Methods According to the results of postoperative pathological diagnosis,62 patients with pulmonary ground glass nodule adenocarcinoma who underwent chest HRCT examination were divided into MIA group and IAC group.The HRCT imaging signs of pulmonary ground glass nodules were compared between the two groups.Results Significant difference showed in nodule type,nodule shape,air bronchogram,vascular convergence sign,pleural indentation sign,nodule size and average CT value between the two groups(P<0.05).Conclusions HRCT signs of pulmonary ground glass nodules are different between MIA and IAC patients.The type,shape,air bronchogram,vascular convergence sign,pleural indentation sign,nodule size and average CT value of pulmonary ground glass nodules have important guiding significance for clinical identification of MIA and IAC.
