摘要
目的探讨经腹与经会阴彩色超声在前置胎盘早期诊断中的应用价值。方法75例疑似前置胎盘孕妇均进行经腹与经会阴彩色超声检查,以产后诊断结果为金标准,分析经腹与经会阴彩色超声与金标准结果的一致性,并比较两种检查方式对前置胎盘的诊断效能。结果75例疑似前置胎盘孕妇确诊为前置胎盘67例。Kappa检验显示,经腹部超声检查结果与产后诊断结果具有一般的一致性(Kappa值=0.592,P<0.05),经会阴超声检查结果与产后诊断结果具有高度的一致性(Kappa值=0.914,P<0.05)。经会阴超声的诊断灵敏度为95.52%、特异性为100.00%、准确率为96.00%,均高于经腹部超声的73.13%、50.00%和70.67%(P<0.05)。结论经会阴彩色超声在前置胎盘早期诊断中的应用价值高,能够为疾病的早期诊断提供准确的数据支持。
Objective To explore the application value of transabdominal and transperineal color ultrasound in the early diagnosis of placenta previa.Methods 75 pregnant women with suspected placenta previa were examined by transabdominal and transperineal color ultrasound.The postpartum diagnosis results were used as the gold standard to analyze the consistency of transabdominal and transperineal color ultrasound with the gold standard results,and to compare the diagnostic efficacy of the two methods for placenta previa.Results Among 75 pregnant women with suspected placenta previa,67 cases were diagnosed as placenta previa.Kappa test showed that the results of transabdominal ultrasound were generally consistent with the results of postpartum diagnosis(Kappa value=0.592,P<0.05),and the results of transperineal ultrasound were highly consistent with the results of postpartum diagnosis(Kappa value=0.914,P<0.05).The diagnostic sensitivity,specificity and accuracy of transperineal ultrasound were 95.52%,100.00%and 96.00%,higher than 73.13%,50.00%and 70.67%of transabdominal ultrasound(P<0.05).Conclusions Transperineal color ultrasound has high application value in the early diagnosis of placenta previa,which can provide accurate data support for the early diagnosis of the disease.
