研究宫腔镜联合LNG-IUS治疗子宫内膜息肉疾病的临床安全性

Clinical Safety of Hysteroscopy Combined with LNG-IUS in the Treat⁃ment of Endometrial Polyps

目的 探讨对子宫内膜息肉疾病患者给予宫腔镜+左炔诺孕酮宫内缓释系统(levonorgestrel-releasing intrauterine system, LNG-IUS)后获得临床效果以及安全性。方法 选取2019年2月—2021年4月山东省菏泽市牡丹人民医院88例子宫内膜息肉患者作为研究对象,以投掷硬币法分组,参照组施以宫腔镜子宫内膜息肉电切术治疗(n=44);研究组在参照组基础上施以LNG-IUS治疗(n=44)。对比两组患者的治疗效果、治疗安全性、疾病复发率、子宫内膜厚度、血红蛋白水平以及月经量水平。结果 研究组子宫内膜息肉患者治疗总有效率(95.45%)高于参照组(79.55%),差异有统计学意义(χ^(2)=4.176,P<0.05);研究组治疗后6、12个月疾病复发率、子宫内膜厚度均低于参照组,差异有统计学意义(P<0.05);治疗后,研究组血红蛋白水平高于参照组,月经量少于参照组,差异有统计学意义(P<0.05);两组在治疗期间均未表现出严重不良反应现象。结论 临床对子宫内膜息肉患者在治疗期间,宫腔镜+LNG-IUS方法的有效应用,可显著提高治疗效果,安全性显著,可降低患者子宫内膜厚度,提升血红蛋白水平,降低月经量。

Objective To investigate the clinical effect and safety of hysteroscopy+LNG-IUS(levonorgestrel releasing intrauterine system)in patients with endometrial polyps after treatment.Methods From February 2019 to April 2021,88 patients with endometrial polyp in Heze Peony People´s Hospital of Shandong Province were selected as the study subjects,divided into two groups by coin toss method,and reference group were treated with hysteroscopic endometrial polyp electrotomy(n=44).The study group was treated with LNG-IUS on the basis of the reference group(n=44).Com‐pared the treatment efficacy,treatment safety,disease recurrence rate,endometrial thickness,hemoglobin levels,and menstrual volume levels between the two groups of patients.Results The total effective rate of endometrial polyp in the study group(95.45%)was higher than that in the reference group(79.55%),the difference was statistically signifi‐cant(χ^(2)=4.176,P<0.05).The disease recurrence rate and endometrial thickness of the study group were lower than those of the reference group at 6 and 12 months after treatment,the difference was statistically significant(P<0.05).After treatment,the hemoglobin level in the study group was higher than that in the reference group,and the men‐strual volume level was lower than that in the reference group,the difference was statistically significant(P<0.05).Both groups showed no serious adverse reactions during the treatment period.Conclusion During the clinical treat‐ment of Endometrial polyp,the effective application of hysteroscopy+LNG-IUS method can significantly improve the treatment effect and safety,reduce the endometrial thickness of patients,increase the hemoglobin level,and reduce the amount of menstruation.
