摘要
目的探讨胰岛素不同注射方式对糖尿病(DM)患者的治疗效果。方法136例DM患者均给予重组人胰岛素注射治疗,采用随机排列法将其分为研究1组和研究2组,各68例。研究1组采用多次皮下注射胰岛素(MSII)治疗方式,研究2组采用胰岛素泵持续皮下注射(CSII)治疗方式。比较两组治疗前后糖代谢指标水平及日均胰岛素用量、血糖指标达标时间。结果治疗前后两组组间空腹血糖(FPG)、餐后2 h血糖(2 h PBG)和糖化血红蛋白(HbA1c)水平比较差异无统计学意义(P>0.05)。治疗后,研究1组FPG为(5.80±1.05)mmol/L、2 h PBG为(7.55±1.13)mmol/L、HbA1c为(6.52±0.84)%,研究2组FPG为(5.76±1.04)mmol/L、2 h PBG为(7.50±1.15)mmol/L、HbA1c为(6.49±0.83)%。两组FPG、2 h PBG及HbA1c水平均低于本组治疗前,差异有统计学意义(P<0.05)。研究2组日均胰岛素用量(35.07±3.26)U少于研究1组的(43.85±3.82)U,血糖指标达标时间(5.56±1.21)d短于研究1组的(10.08±1.54)d,差异有统计学意义(P<0.05)。结论MSII和CSII治疗方式均可以有效控制DM患者血糖水平,但CSII治疗方式相较于MSII可以减少胰岛素的使用剂量,达到快速见效的效果,临床应用价值显著。
Objective To discuss the effect of different insulin infusion methods in diabetes mellitus(DM)patients.Methods A total of 136 cases of DM patients were treated with recombinant human insulin injection and randomly divided into study group 1 and study group 2,with 68 cases in each group.Study group 1 was treated with multiple subcutaneous insulin injections(MSII),and study group 2 was treated with continuous subcutaneous insulin injection(CSII).The level of glucose metabolism index before and after treatment,daily insulin dosage and time of blood glucose reaching the standard were compared between the two groups.Results There were no statistically significant differences in fasting plasma glucose(FPG),2 h postprandial blood glucose(2 h PBG)and glycosylated hemoglobin(HbA1c)levels between the two groups before and after treatment(P>0.05).After treatment,study group 1 had FPG of(5.80±1.05)mmol/L,2 h PBG of(7.55±1.13)mmol/L,and HbA1c of(6.52±0.84)%;study group 2 had FPG of(5.76±1.04)mmol/L,2 h PBG of(7.50±1.15)mmol/L and HbA1c of(6.49±0.83)%.The levels of FPG,PBG and HbA1c in both groups were lower than those before treatment in this group,and the differences were statistically significant(P<0.05).The daily insulin dosage of(35.07±3.26)U in study group 2 was lower than that of(43.85±3.82)U in study group 1;the time of blood glucose reaching the standard of(5.56±1.21)d in study group 2 was shorter than that of(10.08±1.54)d in study group 1;the differences were statistically significant(P<0.05).Conclusion Both MSII and CSII can effectively control the blood glucose level in DM patients,but compared with MSII,CSII can reduce the dosage of insulin and achieve a rapid effect,with significant clinical application value.
