躯体形式障碍的失匹性负波和Quisi脑电变化

Characteristics of mismatched negative waves and Quisi EEG in patients with somatoform disorder

目的:探讨躯体形式障碍(SFD)患者失匹性负波(MMN)及睡眠脑电(Quisi)特点。方法:应用德国脑电生理仪对2019年12月至2022年6月华东师范大学附属精神卫生中心60例SFD患者分别在治疗前和治疗后6周进行MMN和Quisi检测。正常健康人组(NC)36名作为对照。结果:60例SFD中,症状出现频率由高到低依次为认知睡眠、呼吸、消化、五官及心血管症状。与NC组相比,治疗前SFD患者MMN潜伏期延迟,波幅下降(P<0.05或P<0.01)。P300也见同一趋势变化,与此同时Quisi睡眠显示异常(P<0.05或P<0.01),SFD组治疗6周后,MMN和P300若干主成分指标恢复(P<0.05)。夜间Quisi显示睡眠效率[治疗前(85±7%),治疗后(90±4%),P<0.05]提高,觉醒时间和S1减少,S2增加,睡眠潜伏期缩短。结论:Quisi便捷是其临床转化的价值,与MMN联合应用更有助于反映SFD患者的睡眠和认知功能变化。

Objective:To investigate the characteristics of mismatch negativity wave(MMN)and sleep EEG(Quisi)in patients with somatoform disorder(SFD).Method:60 patients with SFD in mental health center affiliated to east China normal university from December 2019 and June 2022 were tested for MMN and Quisi by German electroencephalograph before and 6 weeks after treatment,respectively.36 normal healthy people(NC)were served as controls.Results:Among the 60 cases of SFD included in the study,the symptom frequency from high to low was cognitive function,sleep,respiratory system,digestive system,facial discomfort and cardiovascular system symptoms.Compared with NC group,MMN latency of SFD patients was delayed and the amplitude was decreased before treatment(P<0.05 or P<0.01).P300 also showed the same trend.At the same time,Quisi sleep showed abnormal(P<0.05 or P<0.01).After 6 weeks of treatment,some principal component indexes of MMN and P300 in SFD patients recovered(P<0.05).Quisi at night showed that sleep efficiency[(85±7%)before treatment,(90±4%)after treatment,P<0.05]increased,awakening time and S1 decreased,S2 increased,and sleep latency shortened.Conclusion:The convenience of Quisi is the value of its clinical transformation,and its combined application with MMN is more helpful to reflect the changes of sleep and cognitive function of SFD patients.
