摘要
目的:分析外周血涂片镜检联合纤维蛋白(原)降解产物(FDP)、D-二聚体(D-D)水平在急性早幼粒细胞白血病中的诊断价值。方法:选取2019年1月-2021年8月期间医院收治的急性早幼粒细胞白血病患者48例及急性单核细胞白血病患者25例,在院检查时分别采用外周血涂片镜检配合FDP、D-D水平检测,统计急性早幼粒细胞白血病检出率,评估其诊断价值。结果:联合指标(外周血镜检+FDP+D-D)诊断的灵敏度91.67%、特异度92.00%,外周血镜检(41.67%、52.00%)、FDP(56.00%、87.50%)及D-D(92.00%、70.80%),差异有统计学意义(P<0.05)。病例组的白细胞计数、骨髓早幼粒细胞百分比均高于对照组,血小板计数低于对照组,FDP、D-D水平均高于对照组(P<0.05)。结论:外周血涂片镜检配合FDP、D-D水平诊断急性早幼粒细胞白血病有较高的临床价值,应用意义显著。
Objective:To analyze the diagnostic value of peripheral blood smear combined with fibrinogen degradation products(FDP)and D-dimer(D-D)levels in acute promyelocytic leukemia.Methods:Forty eight patients with acute promyelocytic leukemia and 25 patients with acute monocytic leukemia admitted to the hospital from January 2019 to August 2021 were selected.During the hospital examination,microscopic examination of peripheral blood smear combined with FDP and D-D levels were used to count the detection rate of acute promyelocytic leukemia and evaluate its diagnostic value.Results:The sensitivity and specificity of the combined index(peripheral blood microscopy+FDP+D-D)were 91.67% and 92.00% respectively.The diference between peripheral blood microscopy(41.67%,52.00%),FDP(56.00%,87.50%)and D-D(92.00%,70.80%)was statistically significant(P<0.05).The white blood cell count and percentage of bone marrow promyelocytes in the case group were higher than those in the control group,the platelet count was lower than that in the control group,and the FDP and D-D water were higher than those in the control group on average(P<0.05).Conclusion:Microscopic examination of peripheral blood smear combined with FDP and D-Dlevels has high clinical value in diagnosing acute promyelocytic leukemia,and has significant application significance.
