多层螺旋CT对甲状腺癌及甲状腺腺瘤的临床鉴别诊断价值分析

Analysis of Clinical Differential Diagnosis Value of Multi-Row Spiral CT in Thyroid Carcinoma and Adenoma

目的:分析在甲状腺腺瘤临床鉴别中应用多层螺旋CT检查对良恶性肿瘤的鉴别效果。方法:回顾性分析2020年2月~2022年8月于江西省宜春市万载县人民医院放射科行甲状腺CT检查的患者共计66例。其中经过手术病理检查共确诊甲状腺癌患者36例,甲状腺腺瘤患者30例。患者均应用64排螺旋CT进行扫描。比较检出结果及良恶性肿瘤病灶特征间的差异。结果:CT检查甲状腺癌检出率为83.33%(30/36),甲状腺肿瘤检出率为93.33%(28/30)。CT检查与病理检查结果检出率对比,检测结果符合率为87.88%(58/66)。甲状腺腺瘤与甲状腺癌患者在进行多层螺旋CT检查时,两者影像学征象在密度方面差异并不显著(P>0.05),但是在病灶边缘规则度、囊变程度、是否钙化及强化程度方面,则具有明显的成像特征差异,差异有统计学意义(P<0.05)。结论:应用64排螺旋CT进行甲状腺癌及甲状腺腺瘤的鉴别诊断分析,可通过观察病灶影像特征对患者病变程度进行有效的鉴别诊断,为临床治疗提供依据。

Objective:To analyze the clinical differential effect of multi slice spiral CT(MSCT)in the diagnosis of benign and malignant thyroid adenomas.Methods:A total of 66 patients who underwent thyroid CT examination in the Department of Radiology,Wanzai County People’s Hospital,Yichun City,Jiangxi Province from February 2020 to August 2022 were retrospectively analyzed.A total of 36 patients with thyroid cancer and 30 patients with thyroid adenoma were confirmed by surgical and pathological examination.All patients were scanned using 64-slice spiral CT.Compare the detection results and the differences between benign and malignant tumor focus characteristics.Results:Under CT examination,the detection rate of thyroid cancer was 83.33%(30/36),and the detection rate of thyroid tumors was 93.33%(28/30).Compared with the detection rate of pathological examination results,the coincidence rate of detection results was 87.88%(58/66).When MSCT was performed in patients with thyroid adenoma and thyroid cancer,there was no significant difference in the density of imaging signs between the two groups(P>0.05),but there were significant differences in imaging characteristics in the regularity of the lesion margin,the degree of cystic change,whether there was calcification,and the degree of enhancement,with a statistically significant difference(P<0.05).Conclusion:Using 64-slice spiral CT in the differential diagnosis of thyroid cancer and thyroid adenoma can effectively differentiate the degree of disease in patients by observing the imaging characteristics of the lesions,providing a basis for clinical treatment.
