多层螺旋CT于阑尾炎的临床诊断价值及影像学特征

Clinical Diagnostic Value of Multi-Slice Spiral CT in Appendicitis and Imaging Features

目的:探讨多层螺旋CT(MSCT)在阑尾炎诊断中的临床价值,并对其影像学特点进行分析。方法:回顾性选取2015年1月~2022年12月本院及南通大学附属医院所收治的阑尾炎患者110例为研究对象,所有患者均行MSCT检查及病理学检查。比较病理学检查与MSCT检查的检出结果;比较MSCT检查与病理学检查的诊断效能;进行相关影像学特征分析。结果:110例病理证实的阑尾炎患者经MSCT检查诊断为阳性98例,阴性12例;MSCT检查下的灵敏度、准确度均为89.09%,均处于较高水平,数据结果存在显著差异(P<0.05);病理诊断结果显示,在MSCT为阳性的患者中单纯性阑尾炎的检出率为12.24%(12/98),化脓性阑尾炎检出率为76.53%(75/98),坏疽性阑尾炎检出率为11.22%(11/98),MSCT检查下的单纯性阑尾炎的检出率为15.31%(15/98),化脓性阑尾炎检出率为74.49%(73/98),坏疽性阑尾炎检出率为10.20%(10/98),其分型正确率与病理诊断结果相比无明显差异(P>0.05)。结论:临床上应用MSCT检查可有效检出阑尾炎患者的阳性情况,且具有较高的灵敏度、准确度及正确率,临床上可根据患者自身情况选择相应的检查方式应用于阑尾炎患者的辅助检查,进而对其病情诊断及预后评估起到一定参考价值,为患者的诊疗方案提供依据。

Objective:To evaluate the clinical value of multi-slice spiral CT(MSCT)in the diagnosis of appendicitis and to analyze its imaging features.Methods:A retrospective study was conducted on 110 patients with appendicitis admitted to our hospital and the affiliated hospital of Nantong University from January 2015 to December 2022.All patients underwent MSCT and pathological examinations.Compare the detection results of pathological examination and MSCT examination;Compare the diagnostic efficacy of MSCT examination with pathological examination;Conduct relevant imaging feature analysis.Results:Among 110 patients with pathologically confirmed appendicitis,98 were diagnosed as positive and 12 as negative through MSCT examination;The sensitivity and accuracy under MSCT examination were both 89.09%,both at a high level,with significant differences in data results(P<0.05);The pathological diagnosis results showed that in patients with positive MSCT,the detection rate of simple appendicitis was 12.24%(12/98),the detection rate of suppurative appendicitis was 76.53%(75/98),the detection rate of gangrenous appendicitis was 11.22%(11/98),the detection rate of simple appendicitis under MSCT was 15.31%(15/98),the detection rate of suppurative appendicitis was 74.49%(73/98),and the detection rate of gangrenous appendicitis was 10.20%(10/98),there was no significant difference in the accuracy of its classification compared to the pathological diagnosis results(P>0.05).Conclusion:Clinical application of MSCT can effectively detect the positive condition of appendicitis patients.It has high sensitivity,accuracy and accuracy,and can be used in patients with appendicitis according to their own conditions in order to provide a reference value for the diagnosis and prognosis assessment of the disease,and provide a basis for the diagnosis and treatment of patients.
