摘要
目的:分析甲状腺结节疾病特点,评价彩色多普勒超声观察血流丰富程度情况下的诊断价值。方法:选择2019年8月~2020年8月本院收治的甲状腺结节疾病患者80例,经手术病理结果证实,良性结节与恶性结节分别为60例、20例,分别作为良性组、恶性组,经伦理委员会批准。均行彩超诊断,比较2组甲状腺结节患者的血流丰富程度、不同大小甲状腺结节内血流丰富程度以及甲状腺功能指标、血流动力学参数。结果:良性组与恶性组患者的血流丰富程度1级、2级、3级差异显著,P<0.05;甲状腺结节<1cm、>1cm血流丰富程度差异显著,P<0.05;甲状腺功能指标方面,良性组甲状腺素(Thyroxine,T4)、三碘甲状原氨酸(Triiodothyronine,T3)、促甲状腺激素(thyroidstimulatinghormone,TSH)水平均低于恶性组,P<0.05;血流动力学参数方面,良性组患者的血流阻力指数(Resistance Index,RI)以及收缩期峰值流速(Peak Systolic Velocity,PSV)均低于恶性组,P<0.05。结论:甲状腺结节具有临床多发趋势,通过彩超能够在了解患者血流丰富程度等情况的基础上进行良恶性结节鉴别诊断,有助于疾病早期治疗,提升患者治疗预后效果与安全性。
Objective:To analyze the characteristics of thyroid nodules and evaluate the diagnostic value of color Doppler ultrasound in the presence of blood flow abundance.Methods:A total of 80 patients with thyroid nodule disease admitted to our hospital from August 2019 to August 2020 were selected.The surgical pathological results confirmed that there were 60 cases of benign nodules and 20 cases of malignant nodules,respectively,as The benign group and the malignant group were approved by the ethics committee.Color Doppler ultrasound diagnosis was performed to compare the abundance of blood flow in the two groups of patients with thyroid nodules,the abundance of blood flow in thyroid nodules of different sizes,as well as thyroid function indexes and hemodynamic parameters.Results:The blood flow richness of patients in the benign group and the malignant group was significantly different in level 1,2,and 3,P<0.05;The blood flow richness of thyroid nodules<1cm,>1cm was significantly different,P<0.05;Thyroid function index on the one hand,Thyroxine(T4),Triiodothyronine(T3),and thyroid stimulating hormone(TSH)levels in the benign group were lower than those in the malignant group,P<0.05;Hemodynamic parameters on the one hand,the resistance index(RI)and peak systolic velocity(PSV)of the benign group were lower than those of the malignant group,P<0.05.Conclusion:Thyroid nodules have a clinical tendency to occur frequently.Color Doppler ultrasound can make differential diagnosis of benign and malignant nodules on the basis of understanding the abundance of blood flow in patients,which can help early treatment of the disease and improve the prognostic effect and safety of patients.
