多模态影像学用于早期乳腺癌筛查的研究

摘要 目的:探讨多模态影像学在筛查早期乳腺癌中的价值。方法:选择2020年9月~2022年9月本院收治的疑似早期乳腺癌100例患者纳入本研究,所有患者均进行常规超声(US)、超声造影(CEUS)、弹性成像(UE)及钼靶检查,以病理结果为金标准,分析单一检测和多模态超声检测(联合四项)在早期乳腺癌筛查中的应用价值。结果:100例疑似乳腺癌患者中恶性60例,良性40例;US、CEUS、UE及钼靶的阳性检出率分别为66.67%、80.00%、75.00%及83.33%,均低于联合检测的96.67%(P均<0.05);联合检测的灵敏度、准确度及阴性预测值均显著高于单一检测(P均<0.05);联合检测在浸润性导管癌、导管内原位癌、浸润性小叶癌、髓样癌和黏液腺癌等癌肿鉴别准确率明显高于单一检测(P均<0.05)。结论:多模态影像学用于临床筛查早期乳腺癌具有较高可行性,可以提高早期癌肿检出率和准确鉴别癌肿类型,为其后续医疗工作的有效展开提供科学依据。 Objective:To explore the value of multimodal imaging in screening for early-stage breast cancer.Methods:From September 2020 to September 2022,100 patients with suspected early breast cancer were included in this study.All patients were subjected to routine ultrasound(US),contrast ultrasound(CEUS),elastography(UE)and molybdenum target examination,with pathological results as the gold standard,and analyzed the application value of single detection and multimodal ultrasound detection(combined four)in early breast cancer screening.Results:60 were malignant among 100 suspected breast cancer and 40 were benign;The positive detection rates of US,CEUS,UE and molybdenum were 66.67%,80.00%,75.00%and 83.33%,lower than 96.67%of combined test(P<0.05);The sensitivity,accuracy and negative predictive value were significantly higher than single test(P<0.05);In invasive ductal carcinoma,ductal carcinoma in situ,invasive lobular carcinoma,medullary carcinoma and mucinous adenocarcinoma(P<0.05).Conclusion:Multi-modal imaging has high feasibility for clinical screening of early breast cancer,which can improve the detection rate of early cancer and accurately identify cancer types,and provide scientific basis for the effective development of subsequent medical work.
