摘要
目的:研究女性盆底功能障碍性疾病(PFD)诊断中三维超声的应用价值。方法:选取2021年1月~2022年1月本院收治的67例PFD患者为观察组,同期选取50例健康体检者(盆底功能正常)为对照组,入组者均行二维超声、三维超声检查,以《妇科泌尿学》中PFD诊断标准为金标准,将二维超声、三维超声检查结果与金标准进行比较,分析不同检查方法对PFD的诊断效能,并对两组不同状态下各超声探查指标进行比较。结果:三维超声对PFD诊断准确度、灵敏度及特异度分别为91.45%、88.06%及96.00%,均高于二维超声74.36%、70.15%及80.00%,差异有统计学意义(P<0.05);观察组尿道转移角度、前盆腔膨出距离、子宫脱垂距离及直肠脱垂距离均高于对照组(P<0.05);观察组静息状态、Vslsalva动作下盆膈裂孔横径、前后径均高于对照组,肛提肌厚度低于对照组(P<0.05)。结论:PFD诊断中三维超声的应用价值较高,可清晰显示盆底解剖结构及功能,有利于提高诊断准确度。
Objective:To study the value of three-dimensional ultrasound in the diagnosis of female pelvic floor dysfunction(PFD).Methods:67 patients with PFD treated in our hospital from January 2021 to January 2022 were selected as the observation group,and 50 healthy persons(with normal pelvic floor function)were selected as the control group.All the participants underwent two-dimensional ultrasound and three-dimensional ultrasound.Taking the PFD diagnostic standard in gynecology and urology as the gold standard,the results of two-dimensional ultrasound and three-dimensional ultrasound were compared with the gold standard,and the diagnostic efficiency of different examination methods on PFD was analyzed,the ultrasonic detection indexes of the two groups in different states were compared.Results:The accuracy,sensitivity and specificity of three-dimensional ultrasound in the diagnosis of PFD were 91.45%,88.06%and 96.00%respectively,which were higher than 74.36%,70.15%and 80.00%of two-dimensional ultrasound(P<0.05);The urethral transfer angle,anterior pelvic bulge distance,uterine prolapse distance and rectal prolapse distance in the observation group were higher than those in the control group(P<0.05);The transverse and anteroposterior diameters of the pelvic hiatus in the observation group under resting state and Vslsalva action were higher than those in the control group,while the thickness of the levator ani muscle was lower than that in the control group(P<0.05).Conclusion:Three-dimensional ultrasound is of high value in the diagnosis of PFD.It can clearly show the anatomical structure and function of pelvic floor,which is helpful to improve the accuracy of diagnosis.
