无锡市惠山区剖宫产率及剖宫产指征现状调查

Analysis of Cesarean Section Rate and Indications in Huishan District of Wuxi City

目的 分析无锡市惠山区近年来剖宫产率及剖宫产指征情况,探讨县区级医院有效控制剖宫产率的措施。方法 收集2019—2022年间无锡市惠山区产科年报以及产妇信息,对剖宫产率以及剖宫产指征构成情况进行回顾性分析。结果 2019—2022年惠山区助产机构历年总剖宫产率始终大于40%,2020年总剖宫产率、初产剖宫产率略有下降,但此后均逐年上升,近3年差异有统计学意义(P <0.05)。剖宫产指征构成比排名依次为:瘢痕子宫、胎儿窘迫、头盆不称、胎位异常、孕妇存在严重合并症和并发症、孕妇要求的剖宫产、妊娠巨大儿者。结论 无锡市惠山区的剖宫产率较高,应基于不同机构的自身特点探索并建立严格控制剖宫产的综合管理干预策略。

Objective To analyze the cesarean section rate and indications in Huishan District of Wuxi city in recent years,and to explore the effective measures to control the cesarean section rate in county hospitals.Methods The annual reports of obstetrics and maternal information were collected from 2019 to 2022 in Huishan District of Wuxi City,and the cesarean section rate and the indication composition of cesarean section were retrospectively analyzed.Results From 2019 to 2022,the total cesarean section rate of midwifery institutions in Huishan District was always more than 40%.The total cesarean section rate and primiparous cesarean section rate decreased slightly in 2020,but increased year by year since then,with statistically significant differences in the past three years(P<0.05).The indications of cesarean section were as follows:scarred uterus,fetal distress,cephalopelvic disproportion,abnormal fetal position,severe complications of pregnant women,cesarean section requested by pregnant women,and macrosomia.Conclusion The cesarean section rate in Huishan District of Wuxi is high.Comprehensive management and intervention strategies for strict control of cesarean section should be explored and established based on the characteristics of different institutions.
