CT冠脉成像联合血清IMA、TRPM7在缺血性心肌病诊断中的应用价值 被引量:1

Application Value of CT Coronary Angiography Combined with Serum IMA and TRPM7 in The Diagnosis of Ischemic Cardiomyopathy

摘要 目的分析CT冠脉成像(CTA)联合血清缺血修饰白蛋白(IMA)、瞬时受体电位M7(TRPM7)在缺血性心肌病(ICM)诊断中的应用价值。方法收集2021年2月-2022年2月在我院治疗的ICM患者146例即为ICM组,根据纽约心脏病学会(NYHA)分级分为轻度组45例、中度组54例、重度组47例,入院后行CTA检查;同期选择本院正常体检健康者140例作为对照组。酶联免疫吸附法(ELISA)检测血清IMA、TRPM7的表达水平;采用受试者工作特征曲线(ROC)评估血清IMA、TRPM7水平对ICM的诊断价值。采用四格表评估CTA联合血清IMA、TRPM7水平对ICM的诊断效能。结果与对照组相比,ICM组患者血清IMA、TRPM7水平较高(P<0.05)。重度组的IMA、TRPM7表达水平显著高于中度组和轻度组,中度组高于轻度组(P<0.05)。相关性分析结果显示,ICM组患者血清中IMA的表达水平与TRPM7呈正相关关系(P<0.05)。ROC曲线显示,血清IMA、TRPM7水平联合诊断的AUC高于IMA、TRPM7单独诊断的AUC值(P<0.05);四格表计算显示,CTA联合IMA、TRPM7诊断ICM的准确度为92.18%,其敏感度为98.38%,特异性为38.58%,其中三者联合诊断敏感度高于CTA、IMA、TRPM7单独诊断的敏感度(P<0.05)。结论ICM患者血清中IMA、TRPM7表达水平升高,CTA联合IMA、TRPM7可明显提高ICM诊断的价值。 Objective To analyze the application value of CT coronary angiography(CTA)combined with serum ischemia-modified albumin(IMA)and transient receptor potential M7(TRPM7)in the diagnosis of ischemic cardiomyopathy(ICM).Methods From February 2021 to February 2022,146 patients with ICM who were treated in our hospital were regarded as ICM group,they were grouped into mild group(45 cases),moderate group(54 cases)and severe group(47 cases)according to the New York Heart Association(NYHA)classification,CTA examination was performed after admission;during the same period,140 healthy people with normal physical examination in our hospital were included as the control group.The expression levels of serum IMA and TRPM7 were detected by enzyme-linked immunosorbent assay(ELISA).Receiver operating characteristic curve(ROC)was used to evaluate the diagnostic value of serum IMA and TRPM7 levels for ICM.Four-table table was used to evaluate the diagnostic efficacy of CTA combined with serum IMA and TRPM7 levels for ICM.Results Compared with the control group,the levels of serum IMA and TRPM7 in the ICM group were higher(P<0.05).The expression levels of IMA and TRPM7 in the severe group were obviously higher than those in the moderate and mild groups,and the moderate group were higher than the mild group(P<0.05).The results of correlation analysis showed that the expression level of IMA in serum of patients in the ICM group was positively correlated with TRPM7(P<0.05).The ROC curve showed that the AU C of the combined diagnosis of serum IMA and TRPM7 was higher than the AU C of IMA and TRPM7 alone(P<0.05);the four-table calculation showed that the accuracy of CTA combined with IMA and TRPM7 in diagnosing ICM was 92.18%,its sensitivity was 98.38%,and its specificity was 38.58%,and the combined diagnostic sensitivity of the three was higher than that of CTA,IMA and TRPM7 alone(P<0.05).Conclusion The expression levels of IMA and TRPM7 in serum of ICM patients are increased,and CTA combined with IMA and TRPM7 can obviously improve the diagnostic value of ICM.
