MRI小肠造影在克罗恩病诊断的应用

Application of MRI Enterography in The Diagnosis of Crohn's Disease

目的探究MRI小肠造影在克罗恩病(CD)诊断中的应用价值。方法选择2019年6月~2022年6月期间在我院接受检查的疑似CD患者,经病理结果证实,CD患者共92例,纳入CD组,其余45例均为非CD患者,纳入非CD组。两组均在治疗前接受MRI小肠造影检查,比较两组MRI小肠造影征象,并对有统计差异的MRI小肠造影征象进行Logistic回归分析,明确独立影响指标,并绘制受试者工作曲线(ROC),以曲线下面积(AUC)衡量MRI小肠造影对CD的诊断价值。结果两组肠壁增厚、病变分布、肠腔狭窄、梳齿征、肠系膜脂肪间隙模糊、表观扩散系数(ADC)差异有统计学意义(P<0.05),多因素Logistic回归分析显示肠壁增厚≥3mm、梳齿征是诊断CD发生的影响因素(P<0.05),ROC曲线结果显示两者联合检测诊断价值最高,其AUC为0.917,敏感度、特异度分别为73.48%、97.78%。结论MRI小肠造影诊断CD应用价值较高,可作为CD患者诊断的影像学检查方法。

Objective To explore the application value of MRI enterography in the diagnosis of Crohn's disease(CD).Methods The patients with suspected CD in the hospital were enrolled between June 2019 and June 2022.It was confirmed by surgical resection and pathological results that there were 92 patients with CD in CD group and 45 patients without CD in non-CD group.All underwent MRI enterography before treatment.MRI enterography signs in the two groups were compared,and those with differences were analyzed by Logistic regression analysis.The diagnostic value of MRI enterography for CD was determined by area under receiver operating characteristic(ROC)curve(AU C).Results There were significant differences in intestinal wall thickening,lesion distribution,intestinal lumen stenosis,comb sign,mesenteric fat gap ambiguity and apparent diffusion coefficient(ADC)between the two groups(P<0.05).Multivariate logistic regression analysis showed that intestinal wall thickening≥3 mm and comb sign were the influencing factors in the diagnosis of CD(P<0.05).ROC curve analysis found that the AU C,sensitivity and specificity of combined diagnosis were 0.917,73.48%and 97.78%,respectively.Conclusion MRI enterography has high diagnostic value for CD,which can be applied as imaging examination method for CD patients.
