前庭性偏头痛相关认知障碍发病机制的研究进展

Research Progress on Mechanisms of Cognitive Impairment Associated with Vestibular Migraine

前庭性偏头痛(vestibular migraine,VM)的常见症状是自发性或位置性眩晕发作,伴有偏头痛,认知障碍是容易被忽视的合并症状。VM相关认知障碍与VM的临床症状相互关联,主要涉及记忆、语言、视空间和注意力等功能变化,受年龄、脑血管状态、治疗药物的影响。海马萎缩、脑岛介导的认知-疼痛交互作用,视觉信息处理异常、5-羟色胺和炎症作用等最终可能导致VM相关认知障碍的发生。前庭康复、认知行为疗法和药物可能对VM相关认知障碍有一定的治疗作用。

Common symptoms of vestibular migraine(VM)are spontaneous or positional vertigo,accompanied by migraine,and cognitive impairment is an easily overlooked complication.VM-related cognitive impairment is correlated with clinical symptoms of VM,mainly involving functional changes in memory,language,visual space and attention.It can be influenced by age,cerebrovascular conditions and therapeutic drugs.Hippocampal atrophy,insula-mediated cognitive-pain interactions,abnormal visual information processing,5-hydroxytryptamine and inflammatory effects may eventually lead to the development of VM-related cognitive impairment.Vestibular rehabilitation,cognitive behavioral therapy,and medications may have a therapeutic effect on VM-related cognitive impairment.
