老年高血压患者冠心病相关因素的潜在类别分析

Analysis of potential categories of coronary heart disease related factors in elderly hypertensive patients

目的:采用潜在类别分析法探讨老年高血压患者发生冠心病的影响因素。方法:选取于2019年5月至2022年5月,在本院就诊的高血压患者(235例),根据患者冠状动脉血管管腔狭窄程度,将患者分为冠心病组(64例)和非冠心病组(171例),并对冠心病的影响因素进行单因素分析。通过多因素Logistic回归分析影响患者发生冠心病的危险因素。采用潜在类别分析法比较冠心病高风险组与低风险组间冠心病影响因素分布特征的差异。结果:分析结果显示:年龄、吸烟、饮酒、糖尿病、血脂异常、高血压程度、Gensini积分、LVEF及HDL-C均为患者发生冠心病的危险因素(P<0.05)。潜在类别分类结果显示,与冠心病低风险组相比,冠心病高风险组中“危险因素较多型分布”比例较高(P<0.05)。结论:不同HDL-C水平的老年高血压患者发生冠心病以及冠心病高风险组与低风险组人群中冠心病影响因素的分布特征均有差异。

Objective:Exploring the influencing factors of coronary heart disease in elderly hypertensive patients using latent category analysis.Methods:Hypertension patients(235 cases)who visited our hospital from May 2019 to May 2022.Patients were divided into coronary heart disease group(64 cases)and non coronary heart disease group(171 cases)according to the degree of stenosis of coronary artery lumen.The influencing factors of coronary heart disease were analyzed by single factor analysis.Multivariate Logistic regression analysis on risk factors of coronary heart disease.Potential category analysis was used to compare the distribution characteristics of influencing factors of coronary heart disease between high-risk group and low-risk group.Results:The results showed that age,smoking,alcohol consumption,diabetes,dyslipidemia,degree of coronary artery stenosis,Gensini score,left ventricular ejection fraction and HDLC were all independent risk factors for coronary heart disease(P<0.05).The potential category classification results showed that compared with the low-risk group of coronary heart disease,the high risk group of coronary heart disease had a higher proportion of"multiple risk factors distribution"(P<0.05).Conclusions:There are differences in the distribution of coronary heart disease influencing factors between the elderly hypertensive patients with different HDLC levels and the high-risk and low-risk groups of coronary heart disease.
