摘要
目的分析肺结核(PTB)患儿外周血单个核细胞(PBMC)miRNA-31、miRNA-146a表达水平,以及上述指标与细胞免疫和炎症因子的相关性。方法选取该院2019年2月至2022年2月收治的75例PTB患儿作为观察组,另选取同期健康体检儿童70例作为对照组。比较两组PBMC miRNA-31、miRNA-146a表达水平,外周血T淋巴细胞亚群、血清炎症因子水平。采用Pearson相关分析PBMC miRNA-31、miRNA-146a表达水平与T淋巴细胞亚群、炎症因子的关系。采用受试者工作特征(ROC)曲线分析miRNA-31、miRNA-146a联合检测诊断儿童PTB的效能。结果观察组PBMC miRNA-31、miRNA-146a表达水平高于对照组,差异有统计学意义(P<0.05)。观察组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)比值低于对照组,而CD8^(+)比例高于对照组,差异均有统计学意义(P<0.05)。观察组白细胞介素(IL)-2、IL-6、IL-8水平均高于对照组,差异均有统计学意义(P<0.05)。Pearson相关分析发现,PTB患儿PBMC miRNA-31、miRNA-146a表达水平与CD3^(+)、CD4^(+)比例及CD4^(+)/CD8^(+)比值均呈负相关(P<0.05),与CD8^(+)比例及IL-2、IL-6、IL-8水平均呈正相关(P<0.05)。ROC曲线分析结果显示,PBMC miRNA-31、miRNA-146a表达水平联合检测诊断儿童PTB的曲线下面积均大于单项检测,差异均有统计学意义(P<0.05)。结论儿童PTB PBMC miRNA-31、miRNA-146a均存在异常高表达,与细胞免疫、炎症因子均密切相关。
Objective To analyze the expression levels of miRNA-31 and miRNA-146a in peripheral blood mononuclear cells(PBMC)of children with pulmonary tuberculosis(PTB),and the correlations between the above indexes and cellular immunity,inflammatory factors.Methods A total of 75 children with PTB admitted to Shaanxi Rehabilitation Hospital from February 2019 to February 2022 were selected as the observation group,and 70 healthy children who underwent physical examination during the same period were selected as the control group.The expression levels of miRNA-31 and miRNA-146a in PBMC,peripheral blood T lymphocyte subsets and serum inflammatory factors were compared between the two groups.Pearson correlation analysis was used to determine the relationships between the expression levels of miRNA-31,miRNA-146a in PBMC and peripheral blood T lymphocyte subsets,inflammatory factors.The receiver operating characteristic(ROC)curve was used to analyze the efficacy of combined detection of miRNA-31 and miRNA-146a in the diagnosis of PTB in children.Results The expression levels of PBMC miRNA-31 and miRNA-146a in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The levels of CD3^(+)proportion,CD4^(+)proportion,CD4^(+)/CD8^(+)tatio in the observation group were lower than those in the control group,while the proportion of CD8^(+)was higher than that in the control group,and the differences were statistically significant(P<0.05).The levels of interleukin(IL)-2,IL-6 and IL-8 in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that the expression levels of miRNA-31 and miRNA-146a in PBMC of children with PTB correlated negatively with CD3^(+)proportion,CD4^(+)proportion and CD4^(+)/CD8^(+)ratio(P<0.05),and correlated positively with CD8^(+)proportion,IL-2,IL-6 and IL-8 levels(P<0.05).The results of ROC curve analysis showed that the area under the curve(AUC)of combined detection of miRNA-31 and miRNA-146a expression levels in PBMC for diagnosis PTB in children was greater than that of single detection,the differences were statistically significant(P<0.05).Conclusion The expressions of miRNA-31 and miRNA-146a in PBMC of children with PTB are abnormally high,which relates closely to cellular immunity and inflammatory factors.
