连续检测高密度脂蛋白在预测重症新型冠状病毒感染中的应用

摘要 目的分析连续检测高密度脂蛋白(HDL)在重症新型冠状病毒感染(COVID-19)中的预测效能。方法回顾性分析2020年1-3月该院收治的114例COVID-19患者的病历资料,将所有患者按病情严重程度分为非重症组(75例,普通型)与重症组(39例,重型、危重型);比较两组感染第1天、第7天的HDL水平;另绘制受试者工作特征(ROC)曲线,分析HDL在重症COVID-19中的预测效能。结果重症组感染第1天、第7天的HDL水平分别为(0.65±0.08)、(0.48±0.03)mmol/L,均低于非重症组的(1.03±0.24)、(0.96±0.12)mmol/L,差异有统计学意义(P<0.05);ROC曲线分析结果显示,感染第1天、第7天的HDL预测重症COVID-19患者的曲线下面积(AUC)分别为0.927(95%CI:0.876~0.978)、0.950(95%CI:0.911~0.990)。结论重症COVID-19患者机体内的HDL呈异常低表达状态,连续检测HDL可在一定程度上预测重症COVID-19。 Objective To analyze the predictive efficacy of continuous detection of high-density lipoprotein(HDL)in severe novel coronavirus pneumonia(COVID-19).Methods The medical records of 114 COVID-19 patients admitted to Xinyu People′s Hospital from January 2020 to March 2020 were analyzed retrospectively,and all patients were divided into the non-critical group(75 cases,common type)and the critical group(39 cases,severe or critical type)according to the severity of disease.HDL level was compared between the two groups on day 1 and day 7 of infection.Receiver operating characteristic(ROC)curve was also drawn to analyze the predictive efficacy of HDL in severe COVID-19.Results The levels of HDL in the critical group were(0.65±0.08)and(0.48±0.03)mmol/L on day 1 and day 7 of infection respectively,which were lower than those in the non-critical group(1.03±0.24)and(0.96±0.12)mmol/L,with statistical significance(P<0.05).ROC curve analysis results showed that the areas under the curve(AUC)predicted by HDL on day 1 and day 7 for severe COVID-19 patients were 0.927(95%CI:0.876-0.978)and 0.950(95%CI:0.911-0.990)respectively.Conclusion HDL expression is abnormally low in patients with severe COVID-19,and continuous HDL detection can predict severe COVID-19 to a certain extent.
