脓毒症患儿血清FABP3、Presepsin、sTREM-1水平及相关性研究

摘要 目的探讨脓毒症患儿血清心脏型脂肪酸结合蛋白(FABP3)、可溶性白细胞分化抗原14亚型(Presepsin)、可溶性髓样细胞触发受体-1(sTREM-1)水平及3项指标之间的相关性。方法选取2021年3月至2022年3月于该院接受治疗的脓毒症患儿80例纳入脓毒症组,另选取同期于该院进行健康体检的健康儿童80例作为对照组。采用酶联免疫吸附试验检测两组血清FABP3、Presepsin及sTREM-1水平,分别对不同严重程度脓毒症患儿、是否出现休克脓毒症患儿、是否出现心肌损伤脓毒症患儿血清FABP3、Presepsin、sTREM-1水平进行比较。对3项指标的相关性进行分析。结果脓毒症组血清FABP3、Presepsin、sTREM-1水平高于对照组,差异有统计学意义(P<0.05)。重度脓毒症患儿血清FABP3、Presepsin、sTREM-1水平高于轻度脓毒症患儿,差异有统计学意义(P<0.05)。发生休克脓毒症患儿血清FABP3、Presepsin、sTREM-1水平高于未发生休克脓毒症患儿,差异有统计学意义(P<0.05)。发生心肌损伤脓毒症患儿血清FABP3、Presepsin、sTREM-1水平高于未发生心肌损伤脓毒症患儿,差异有统计学意义(P<0.05)。脓毒症患儿血清FABP3水平与Presepsin、sTREM-1水平呈正相关(r=0.358、P<0.001,r=0.362、P<0.001);脓毒症患儿血清Presepsin水平与sTREM-1水平呈正相关(r=0.382,P<0.001)。结论脓毒症患儿血清FABP3、Presepsin、sTREM-1水平异常升高,并且与脓毒症患儿病情严重程度、是否发生休克、是否发生心肌损伤有关,脓毒症患儿血清FABP3水平与Presepsin、sTREM-1水平呈正相关。 Objective To investigate the levels and correlations of cardiac fatty acid-binding protein(FABP3),soluble leukocyte differentiation antigen 14 subtype(Presepsin)and soluble myeloid cell trigger receptor-1(sTREM-1)in children with sepsis.Methods A total of 80 children with sepsis who received treatment in Xingtai People′s Hospital from March 2021 to March 2022 were selected into the sepsis group,and 80 healthy children who underwent physical examination in this hospital during the same period were selected as the control group.The levels of FABP3,Presepsin and sTREM-1 were detected by enzyme-linked immunosorbent assay,and the serum levels of FABP3,Presepsin and sTREM-1 were compared in children with different severity of sepsis,children with or without shock sepsis and children with or without myocardial damage sepsis.The correlations among the three indicators were analyzed.Results Serum FABP3,Presepsin and sTREM-1 levels in sepsis group were higher than those in control group,and the differences were statistically significant(P<0.05).The levels of FABP3,Presepsin and sTREM-1 in children with severe sepsis were higher than those in children with mild sepsis,the differences were statistically significant(P<0.05).The serum levels of FABP3,Presepsin and sTREM-1 in children with septic shock were higher than those in children without septic shock,the differences were statistically significant(P<0.05).The levels of FABP3,Presepsin and sTREM-1 in children with myocardial injury sepsis were higher than those in children without myocardial injury sepsis,the differences were statistically significant(P<0.05).Serum FABP3 level correlated positively with Presepsin and sTREM-1 levels in children with sepsis(r=0.358,P<0.001;r=0.362,P<0.001).Serum Presepsin level correlated positively with sTREM-1 level in children with sepsis(r=0.382,P<0.001).Conclusion The serum levels of FABP3,Presepsin and sTREM-1 in children with sepsis elevate abnormally,and correlate with the severity of the disease,whether shock occurs,and whether myocardial injury occurs in children with sepsis.The serum levels of FABP3,Presepsin and sTREM-1 correlate positively in children with sepsis.
