勾头缩喉法训练对食管癌术后患者吞咽功能及营养状况的影响

Effects of chin-down-plus-larynx-tighten maneuver on swallowing function and nutritional status of patients with esophageal cancer after operation

目的:探讨勾头缩喉法训练对食管癌术后患者吞咽功能以及营养状况的影响。方法:选取91例食管癌术后患者为研究对象,按照随机数字表法分为A组和B组,A组46例给予勾头缩喉法进行吞咽训练,B组45例给予常规吞咽功能训练,观察两组吞咽功能、消化道症状、营养状况以及并发症发生率。结果:干预结束后,A组吞咽功能好于B组,差异有统计学意义(P<0.05)。A组反流、咳嗽、吞咽困难、疲劳、疼痛评分均低于B组(P<0.05)。A组前白蛋白(PA)、转铁蛋白(TF)及白蛋白(ALB)均高于B组(P<0.05)。A组并发症发生率6.52%低于B组24.44%(P<0.05)。结论:勾头缩喉法训练用于食管癌术后吞咽困难患者康复,能提高患者吞咽功能,可减轻消化道症状,改善营养状况,增强康复效果。

Objective:To investigate the effect of chin-down-plus-larynx-tighten maneuver on swallowing function and nutritional status of patients with esophageal cancer after operation.Methods:91 patients with esophageal cancer after surgery were selected as the research objects,and divided into Group A and Group B according to random number table method.46 patients in Group A were given swallowing training with chin-down-plus-larynx-tighten maneuver,while 45 patients in Group B were given routine swallowing function training.The swallowing function,digestive tract symptoms,nutritional status and complication rate were observed between the two groups.Results:The swallowing function of Group A was better than that of Group B after intervention.The difference was statistically significant(P<0.05).The scores of reflux,cough,dysphagia,fatigue and pain in Group A were lower than those in Group B(P<0.05).Prealbumin(PA),trabsferring(TF) and albumin(ALB) in Group A were higher than those in Group B(P<0.05).The complication rate of Group A was 6.52% lower than that of Group B(24.44%)(P<0.05).Conclusion:The application of chin-down-plus-larynx-tighten maneuver in patients with dysphagia after esophageal cancer surgery can improve the swallowing function,reduce gastrointestinal symptoms,improve nutritional status and enhance the rehabilitation effect.
