中西医结合远程康复指导稳定型慢性阻塞性肺疾病患者疗效观察

目的评估中西医结合远程康复指导稳定型慢性阻塞性肺疾病(COPD)患者的疗效。方法COPD患者210例,随机分为两组。对照组:仅药物治疗;观察组:采用药物治疗结合远程指导中西医康复方案。采用健身气功八段锦为基础的有氧训练结合肌肉放松训练、咳嗽和呼吸训练。5次/周,20~30 min/次。由治疗师远程指导实施,治疗持续12周。第4、12周各评估1次。结果治疗4周后,观察组改良Bathel指数评分与对照组比较,差异有统计学意义(P<0.05)、治疗12周后观察组6 min步行试验距离、改良Bathel指数评分、心理异常患者数量、圣·乔治呼吸生活质量调查问卷(SGRQ)评分与对照组比较,差异均有统计学意义(P<0.05)。结论以远程指导为方式,以八段锦结合有氧、咳嗽和呼吸训练等中西医结合康复治疗,稳定型COPD患者在生理、心理、日常生活生活能力及生活质量方面得到提高。

Objective To evaluate the efficacy of combined telerehabilitation in patients with stable chronic obstructive pulmonary disease(COPD).Methods A randomized controlled study design randomly divided 210 COPD patients into two groups.The control group received drug therapy only.The rehabilitation group received drug therapy combined with remote guidance of Chinese and Western rehabilitation programs.The program used the aerobic training based on the fitness Qigong Baduanjin created by the General Administration of Sport of China,combined with muscle relaxation training,cough and breathing training,with 5 sessions per week for 20~30 minutes.The patient was contacted by the therapist once a week for remote guidance on implementation.The treatment was continued for 12 weeks.And patients were evaluated once each time in weeks 4 and 12.Results After 4 weeks of treatment,there was a statistically significant difference(P<0.05)between the observation group and the control group in the modified Bathel index score.After 12 weeks of the treatment,the observation group had a 6-minute walking test distance,a modified Bathel index score,the number of patients with psychological abnormalities,and the St.George's Respiratory Quality of Life Questionnaire(SGRQ)score.The difference was also statistically significant(P<0.05).Conclusion By means of remote guidance,Baduanjin qigong combined with aerobic,cough,respiratory training and other integrated traditional and Western medicine rehabilitation treatment,stable COPD patients have improved in physiology,psychology,daily living ability and quality of life.
