泼尼松联合泛昔洛韦对带状疱疹患者细胞因子水平及疼痛的影响

Effect of Prednisone Combined with Famciclovir on Cytokine Level and Pain in Patients with Herpes Zoster

目的探讨泼尼松联合泛昔洛韦对带状疱疹患者细胞因子水平及疼痛的影响。方法选择2020年3月至2022年3月莆田市皮肤病防治院收治的104例带状疱疹患者,随机分为两组,各52例。对照组采用泛昔洛韦治疗,基于此,观察组加用泼尼松治疗。比较两组治疗效果、症状改善时间、细胞因子水平、疼痛程度以及药物安全性。结果观察组治疗总有效率为96.15%(50/52)高于对照组的80.77%(42/52)(P<0.05)。观察组水疱消退时间为(1.79±0.37)d、止痛时间为(1.65±0.73)d、结痂时间为(4.19±1.04)d、完全愈合时间为(10.67±2.11)d,均短于对照组的(3.08±0.81)d、(2.68±1.02)d、(6.48±1.87)d、(14.86±3.95)d(P<0.05)。两组治疗后IL-2水平均升高,IL-10、疼痛评分均降低,且观察组IL-2水平更高,IL-10、疼痛评分更低(P<0.05)。两组均未出现明显不良反应。结论带状疱疹患者采用泼尼松联合泛昔洛韦治疗可提升疗效,缩短各症状缓解时间,改善细胞因子,降低疼痛程度,加快患者康复,且安全性较高,值得临床广泛应用。

Objective To investigate the effect of prednisone combined with famciclovir on cytokines level and pain in patients with herpes zoster.Methods 104 patients with herpes zoster admitted to Putian Skin Disease Prevention and Control Hospital from March 2020 to March 2022 were randomly divided into two groups,52 patients in each group.The control group was treated with famciclovir,the observation group was treated with prednisone based on control group.The treatment effect,symptom improvement time,cytokine level,pain degree and drug safety were compared between the two groups.Results The total effective rate of treatment in the observation group was 96.15%(50/52),which was higher than that in the control group[80.77%(42/52)],and the difference was statistically significant(P<0.05).The time of blister regression in the observation group was(1.79±0.37)d,the time of pain relief was(1.65±0.73)d,the time of scab formation was(4.19±1.04)d,and the time of complete healing was(10.67±2.11)d,which were shorter than those in the control group(3.08±0.81)d,(2.68±1.02)d,(6.48±1.87)d,(14.86±3.95)d(P<0.05).After treatment,the level of IL-2 increased and the score of IL-10 and pain decreased in both groups,and the level of IL-2 was higher and the score of IL-10 and pain was lower in the observation group,the difference was statistically significant(P<0.05).There were no obvious adverse reactions in both groups.Conclusion Prednisone combined with famciclovir can improve the efficacy of herpes zoster patients,shorten the remission time of symptoms,improve cytokines,reduce the degree of pain,and speed up the recovery of patients,with high safety,which is worthy of wide clinical application.
