腹腔镜下输卵管抽芯切除术联合宫角缝扎治疗输卵管妊娠的临床效果

Clinical Effect of Laparoscopic Salpingectomy by Core Pulling Combined with Cornual Ligation in Treatment of Tubal Pregnancy

目的:探讨腹腔镜下输卵管抽芯切除术联合宫角缝扎治疗输卵管妊娠的临床效果。方法:选取2021年1月—2022年4月毕节市中医医院收治的输卵管妊娠患者82例作为研究对象,按照随机数字表法分为研究组与对照组,各41例。对照组采取输卵管下妊娠病灶清除术,研究组实施腹腔镜下输卵管抽芯切除术联合宫角缝扎。比较两组围术期指标、血清β-人绒毛膜促性腺激素(β-HCG)水平、并发症发生情况。结果:研究组手术时间、术后排气时间、术后下床时间、住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.001)。术前,两组β-HCG水平比较,差异无统计学意义(P>0.05);术后1、3、7 d,研究组β-HCG水平均低于对照组,差异有统计学意义(P<0.001)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:腹腔镜下输卵管抽芯切除术联合宫角缝扎治疗输卵管妊娠的临床效果显著,具有手术时间短、术中出血量少、术后恢复快等优势,可降低血清β-HCG水平,且术后并发症发生率较低。

Objective:To investigate the clinical effect of laparoscopic salpingectomy by core pulling combined with cornual ligation in the treatment of tubal pregnancy.Methods:A total of 82 patients with tubal pregnancy admitted to Bijie Traditional Chinese Medicine Hospital from January 2021 to April 2022 were selected as the study subjects.According to the method of random number table,the patients were divided into study group and control group,with 41 patients in each group.The control group was treated by pregnancy disease focus clearance under the fallopian tube,while the study group was treated by laparoscopic salpingectomy by core pulling combined with cornual ligation.The perioperative indicators,serum β-human chorionic gonadotropin(β-HCG) level,and complications were compared between two groups.Results:The operation time,postoperative exhaust time,postoperative ambulation time,and length of hospital stay in the study group were all shorter than those in the control group,and the intraoperative blood loss in the study group was less than that in the control group,and the difference was statistically significant(P<0.001).Before surgery,there was no significant difference in the levels of β-HCG between the two groups(P>0.05).At 1,3 and 7 d after surgery,the levels of β-HCG in the study group were lower than those in the control group,and the difference was statistically significant(P<0.001).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:The clinical effect of laparoscopic salpingectomy by core pulling combined with cornual ligation is significant in the treatment of tubal pregnancy,which has the advantages of short operation time,low intraoperative blood loss,and fast postoperative recovery,and can reduce serum β-HCG levels and have a low incidence of postoperative complications.
