家庭医生签约管理模式在老年2型糖尿病合并高尿酸血症患者中的应用效果观察

Application Effect Observation of Family Doctor Contract Management Mode in Elderly Patients with Type 2 Diabetes Mellitus Combined with Hyperuricemia

目的:探讨家庭医生签约管理模式在老年2型糖尿病合并高尿酸血症患者的应用效果。方法:选取2020年1月—2022年1月在上海市黄浦区小东门街道社区卫生服务中心参加免费体检的老年2型糖尿病合并高尿酸血症患者92例作为研究对象,采用随机数字表法分为观察组与对照组,各46例。观察组实施家庭医生签约管理,对照组实施常规管理。比较两组干预前后空腹血糖(FBG)、糖化血红蛋白(HbA1c)、血尿酸(SUA)水平及自我管理能力。结果:干预前,两组SUA、FBG、HbA1c水平比较,差异无统计学意义(P>0.05);干预后,观察组SUA、FBG、HbA1c水平均低于对照组,差异有统计学意义(P<0.001)。干预前,两组饮食控制、适度运动、自我监测、遵医行为、定期复查评分比较,差异无统计学意义(P>0.05);干预后,观察组饮食控制、适度运动、自我监测、遵医行为、定期复查评分均高于对照组,差异有统计学意义(P<0.001)。结论:家庭医生签约管理模式在老年2型糖尿病合并高尿酸血症患者中的应用效果显著,可改善患者SUA及血糖水平,提高其自我管理能力,值得应用并予以推广。

Objective:To explore the application effect of family doctor contract management mode on elderly patients with type 2diabetes mellitus combined with hyperuricemia.Methods:A total of 92 elderly patients with type 2 diabetes mellitus and hyperuricemia who participated in free physical examinations in Shanghai Huangpu District Xiaodongmen Street Community Health Service Center from January 2020 to January 2022 were selected as the study subjects.They were divided into control group and observation group according to random number table method,with 46 cases in each group.The observation group implemented family doctor contract management,and the control group implemented routine management.The fasting blood glucose(FBG),glycated hemoglobin(HbA_(1c)) and serum uric acid(SUA) levels,as well as self-management ability were compared between two groups before and after management.Results:Before the intervention,there were no significant differences in SUA,FBG and HbA_(1c) levels between the two groups(P>0.05).After the intervention,the levels of SUA,FBG and HbA_(1c)in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.001).Before the intervention,there were no significant differences in dietary control,moderate exercise,self-monitoring,medical compliance behavior,and regular review scores between the two groups(P>0.05).After the intervention,the dietary control,moderate exercise,self-monitoring,medical compliance behavior,and regular review scores in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.001).Conclusion:The family doctor contract management mode has a significant application effect in elderly patients with type 2 diabetes mellitus and hyperuricemia,which can improve SUA and blood glucose levels,and improve their self-management ability,which is worthy of application and promotion.
