多层螺旋CT在妇科急腹症诊断中的临床探讨

Clinical study of multi-slice spiral CT in the diagnosis of gynecological acute abdomen

目的探讨多层螺旋CT(MSCT)在妇科急腹症诊断中的临床价值。方法选取60例疑似妇科急腹症患者为研究对象,患者采用多层螺旋CT探查,包括黄体破裂、卵巢囊肿破裂、卵巢囊肿蒂扭转、子宫破裂及宫外孕等病症,以手术病理检查为金标准,观察多层螺旋CT的确诊率。结果60例疑似妇科急腹症患者中,经临床手术病理诊断56例确诊为妇科急腹症,其他4例(2例急性阑尾炎,2例肠梗阻)。妇科急腹症包括黄体破裂10例,卵巢囊肿破裂12例,卵巢囊肿蒂扭转11例,盆腔脓肿15例,宫外孕8例;多层螺旋CT检查有54例[54/56(96.43%)]确诊为妇科急腹症患者,其中黄体破裂10例,卵巢囊肿破裂11例,卵巢囊肿蒂扭转11例,盆腔脓肿14例,宫外孕8例,比较差异无统计学意义(P>0.05)。结论多层螺旋CT检测妇科急腹症有较高的确诊率,具有安全、便捷的辅助检测特点,是探查急腹症最有效的影像诊断方法,有助于临床治疗的选择,值得临床推广与应用。

Objective To investigate the clinical value of multi-slice spiral CT(MSCT)in the diagnosis of gynecological acute abdomen.Methods 60 patients with suspected gynecological acute abdomen were selected as the study objects.The patients were explored by multi-slice spiral CT,including luteal rupture,ovarian cyst rupture,torsion of ovarian cyst pedicle,uterine rupture and ectopic pregnancy,etc.The surgical pathological examination was used as the gold standard to observe the diagnosis rate of multi-slice spiral CT.Results Of the 60 suspected patients with gynecological acute abdomen,56 were confirmed as gynecological acute abdomen by clinical operation and pathological diagnosis,and the other 4(2 acute appendicitis and 2 intestinal obstruction).Gynecological acute abdomen included luteal rupture in 10 cases,ovarian cyst rupture in 12 cases,ovarian cyst pedicle torsion in 11 cases,pelvic abscess in 15 cases,ectopic pregnancy in 8 cases.There were 54 patients[54/56(96.43%)]diagnosed with gynecological acute abdomen by multi-slice spiral CT examination,including 10 cases of corpus luteum rupture,11 cases of ovarian cyst rupture,11 cases of ovarian cyst torsion,14 cases of pelvic abscess,and 8 cases of ectopic pregnancy,with no statistical significance(P>0.05).Conclusion Multi-slice spiral CT has a high diagnosis rate for the detection of gynecological acute abdomen,and has the characteristics of safe and convenient auxiliary detection.It is the most effective imaging diagnosis method for the detection of acute abdomen,which is helpful to the selection of clinical treatment,and is worthy of clinical promotion and application.
