循证护理管理对妊娠期糖尿病患者血糖波动情况和母婴结局的影响分析

Analysis of the influence of evidence-based nursing management on blood glucose fluctuation and maternal and infant outcomes in patients with gestational diabetes mellitus

目的探究循证护理管理对妊娠期糖尿病患者血糖波动情况和母婴结局的影响。方法选取54例妊娠期糖尿病患者,按照信封法分为对照组与观察组,每组27例。对照组实施常规护理,观察组实施循证护理管理模式。比较两组患者血糖指标、产后抑郁和糖尿病自护行为状态及不良母婴结局。结果观察组空腹血糖、餐后2h血糖、糖化血红蛋白水平均较对照组低(P<0.05)。观察组爱丁堡产后抑郁量表(EPDS)评分低于对照组,糖尿病自护行为量表(SDSCA)评分高于对照组(P<0.05)。观察组不良母婴结局发生率7.41%较对照组的33.33%低(P<0.05)。结论循证护理管理模式应用在妊娠期糖尿病患者中效果显著,可有效控制血糖异常波动状态,改善抑郁心理,增强患者自护行为,改善母婴结局。

Objective To exploretheinfluenceof evidence-based nursingmanagement on blood glucosefluctuation and maternal and infant outcomes in patients with gestational diabetes mellitus.Methods 54 patients with gestational diabetes mellitus were divided into control group and observation group according to envelope method,with 27 cases in each group.Control group implemented routine nursing,observation group implemented evidence-based nursing management mode.Blood glucose index,postpartum depression,diabetes self-care behavior and adverse maternal and infant outcomes were compared between the two groups.Results The levels of fasting blood glucose,2 h postprandial blood glucose and HBAlc in observation group were lower than those in control group(P<0.05).The edinburgh postpartum depression scale(EPDS)score of the observation group was lower than that of the control group,and the diabetes self-care behavior scale(SDSCA)score was higher than that of the control group(P<0.05).The incidence of adverse maternal and infant outcomes in the observation group was 7.41%lower than that in the control group(33.33%)(P<0.05).Conclusion The application of evidence-based nursing management model in patients with gestational diabetes mellitus has a significant effect,which can effectively control the abnormal fluctuation state of blood sugar,improve depression,enhance patients'self-care behavior,and correct maternal and infant outcomes.
