剖宫产术后实施腹横筋膜神经阻滞麻醉的镇痛效果分析

Analysis of analgesic effect of abdominal transverse fascia nerve block anesthesia after cesarean section

目的研究剖宫产术后实施腹横筋膜神经阻滞麻醉的镇痛效果。方法选择60例接受剖宫产术分娩的产妇,依据随机数字表法分为对照组与研究组,各30例。对照组给予镇痛泵镇痛,研究组实施腹横筋膜神经阻滞麻醉进行镇痛处理。采用数字评分法(NRS)评估两组产妇剖宫产后2h、4h、12h与24h的疼痛程度,并采用舒适状况量表(GCQ)调查两组产妇剖宫产术后的身体舒适性,观察并记录两组产妇剖宫产术后的不良反应发生情况。结果研究组产妇剖宫产后2h、4h、12h与24h的NRS评分分别为(1.38±0.86)、(2.19±0.83)、(2.08±0.68)、(2.19±0.25)分,均低于对照组产妇术后各时刻评分(P<0.05);研究组产妇剖宫产术后的生理、心理、环境与社会生活等舒适度评分分别为(13.28±2.37)、(28.27±3.88)、(19.36±2.85)、(23.07±3.14)分,均高于对照组产妇术后各维度评分(P<0.05);研究组产妇的不良反应发生率为6.67%,低于对照组产妇的26.67%(P<0.05)。结论腹横筋膜神经阻滞麻醉在剖宫产术后产妇镇痛中可取得理想的镇痛效果,并减少产妇不良反应,提升其术后舒适性与配合度,值得应用。

Objective To study the analgesic effect of abdominal transverse fascia nerve block anesthesia after cesarean section.Methods A total of 60 women who received cesarean section were selected and divided into the control group and the study group according to random number table method,with 30 cases in each group.The control group was given analgesia pump,and the study group was given transversal abdominal fascia nerve block anesthesia.The degree of pain 2 h,4 h,12 h and 24 h after cesarean section was assessed by numerical rating system(NRS),and the physical comfort of the two groups was investigated by using the general comfort questionnaire(GCQ),and the occurrence of adverse reactions after cesarean section of the two groups was observed and recorded.Results The NRS scores at 2 h,4 h,12 h and 24 h after cesarean section in the study group were(1.38±0.86),(2.19±0.83),(2.08±0.68)and(2.19±0.25)points,respectively,which were lower than those in the control group(P<0.05).The scores of physiological,psychological,environmental and social comfort after cesarean section in the study group were(13.28±2.37),(28.27±3.88),(19.36±2.85)and(23.07±3.14)points,respectively,which were higher than those in the control group(P<0.05).The incidence of adverse reactions in the study group was 6.67%,which was lower than 26.67%in the control group(P<0.05).Conclusion The abdominal transverse fascia nerve block anesthesia can achieve the ideal analgesic effect in the maternal analgesia,reduce the maternal adverse reactions,and improve its postoperative comfort and coordination degree.It is worthy of application.
