平原地区不同CO_(2)气压对行腔镜切除术乳腺癌患者呼吸循环的影响

摘要 目的 探讨在平原地区使用不同CO_(2)气压对行腔镜切除术乳腺癌患者呼吸循环的影响。方法 选取2019年12月至2022年5月海南医学院第一附属医院收治的在全身麻醉下行单孔腔镜切除术的乳腺癌患者62例,采用随机数字表法分为试验组(8mmHg,1mmHg=0.133kPa,n=28)和对照组(12mmHg,n=34)。比较充气前两组患者的平均动脉压(mean artery pressure,MAP)和手术时间,以及手术充气前后两组患者心率(heart rate,HR)、MAP和呼气末二氧化碳分压(partial pressure of end-tidal carbon dioxide pressure,P_(ET)CO_(2))水平。结果 充气前两组患者的MAP和手术时间比较,差异均无统计学意义(P>0.05)。充气前20min和放气后20min,两组患者的HR、MAP和P_(ET)CO_(2)比较,差异均无统计学意义(P>0.05);充气20min时,试验组的P_(ET)CO_(2)低于对照组,差异有统计学意义(P<0.05)。结论 在平原地区行腔镜乳腺癌切除术,CO_(2)气压越高对呼吸影响越明显。 Objective To investigate the effects of using different CO_(2)pressures on the respiratory cycle of patients undergoing endoscopic resection for breast cancer in the plains.Methods A total of 62 breast cancer patients admitted to the First Affiliated Hospital of Hainan Medical University under general anaesthesia who underwent single-hole endoscopic resection from December 2019 to May 2022 were selected and divided into the experimental group(8mmHg,1mmHg=0.133kPa,n=28)and the control group(12mmHg,n=34)according to the random number table method.The mean artery pressure(MAP)and operation time of the two groups before inflation were compared,as well as the heart rate(HR),MAP and partial pressure of end-tidal carbon dioxide pressure end-tidal carbon dioxide(P_(ET)CO_(2))levels of the two groups before and after surgical inflation.Results Compared with the MAP and operation time of the two groups before inflation,the differences were not statistically significant(P>0.05).At 20 minutes before inflation and 20 minutes after deflation,the HR,MAP and P_(ET)CO_(2)of the two groups of patients were compared,and the differences were not statistically significant(P>0.05).At 20 minutes of inflation,the P_(ET)CO_(2)of the experimental group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The respiratory effects of higher CO_(2)air pressure are more pronounced when performing endoscopic breast cancer resection in the plains.
