摘要
目的 采用Meta分析法分析益生菌补充剂对妊娠期糖尿病孕妇的影响。方法 系统检索PubMed、Embase、Web of Science、EBSCO和Cochrane Library数据库,纳入评估益生菌补充剂对妊娠期糖尿病孕妇影响的随机对照试验。2名研究人员独立搜索文献,提取数据,并评估纳入研究的质量。检索时限均从建库至2021年11月。Meta分析采用固定效应或随机效应模型。结果 纳入随机对照试验6项共830例患者。与对照组相比,益生菌组空腹血清胰岛素(SMD=-0.95,95%CI-1.73~-0.17,P=0.02)、胰岛素抵抗指数(SMD=-1.12,95%CI-2.05~-0.18,P=0.02)显著降低,对空腹血糖(SMD=-0.49,95%CI-1.05~0.08,P=0.09)、胎龄(SMD=0.07,95%CI-0.20~0.34,P=0.63)和妊娠体质量(SMD=-0.11,95%CI-0.38~0.16,P=0.43)无影响。结论 益生菌补充剂可显著降低妊娠期糖尿病患者空腹血清胰岛素及HOMA-IR,但对空腹血糖、胎龄和妊娠体质量无明显影响。
Objective To analyze the effect of probiotic supplements on pregnant women with diabetes during pregnancy by Meta-analysis method.Methods Systematically retrieved PubMed,Embase,Web of Science,EBSCO and Cochrane Library databases,and included randomized controlled trials to evaluate the impact of probiotic supplements on pregnant women with gestational diabetes mellitus.Two researchers independently searched articles,extracted data,and assessed the quality of included studies.The search deadline was from the establishment of the database to November 2021.Meta-analysis used fixed or random effects models.Results Six randomized controlled trials involving 830 patients in the Meta-analysis.Compared with the control group,the probiotic supplementation intervention was found to significantly reduce fasting serum insulin(SMD=-0.95,95%CI-1.73~-0.17,P=0.02)and HOMA-IR(SMD=-1.12,95%CI-2.05~-0.18,P=0.02),but had no substantial effect on fasting blood glucose(SMD=-0.49,95%CI=-1.05~0.08,P=0.09),gestational age(SMD=0.07,95%CI-0.20~0.34,P=0.63)and gestational weight(SMD=-0.11,95%CI-0.38~0.16,P=0.43).Conclusion Probiotic supplements can significantly reduce fasting serum insulin and HOMA-IR,but has no significant effect on fasting blood glucose,gestational age and gestational weight.
