北京市某社区卫生服务中心治疗冠心病的中成药使用情况分析

Analysis on the use of Chinese patent medicines in the treatment of coronary heart disease in a community health service center in Beijing

目的 了解北京市某社区卫生服务中心治疗冠心病的中成药使用现状,为临床合理用药提供依据。方法 从北京市西城区展览路社区卫生服务中心医院信息系统(HIS)中调取相关数据,分析2021年治疗冠心病的中成药销售金额、用药频度(DDDs)、日均费用(DDDc),并针对不合理处方中存在的问题进行整理、归纳。结果 该中心常用治疗冠心病的中成药33种,销售金额居前3位的分别是脑心通胶囊、稳心颗粒、速效救心丸;DDDs居前3位的分别是复方丹参滴丸、速效救心丸、脑心通胶囊;DDDc按升序排列居前3位的分别是心可舒片、黄杨宁片、银杏叶片;不合理处方主要问题是辨证不明、重复用药。结论 该中心用于治疗冠心病的中成药种类较多,绝大多数使用规范,但存在缺少中医辨证、重复用药等不合理现象,应加强全科医师相关知识培训,完善信息系统,优化品种结构,进一步加强合理用药。

Objective To understand the current status of the use of drugs for coronary heart disease in a Community Health Service Center in Beijing,and to provide evidence for their rational clinical application.Methods Relevant data were retrieved from the HIS of Zhanhan Road Community Health Service Center,Xicheng District,Beijing,to analyze the sales amount,frequency of medication(DDDs),and average daily cost(DDDc)of Chinese patent medicines for coronary heart disease in 2021.The problems existing in the rational prescription are sorted and summarized.Results There are 33 Chinese patent medicines commonly used in the center to treat coronary heart disease.The top three in sales amount are Naoxintong Capsules,Wenxin Granules and Suxiao Jiuxin Pills.The top three DDDs are Fufang Danshen Dripping Pills and Suxiao Jiuxin Pills.Pills,Naoxintong Capsules.DDDc ranked the top 3 in ascending order:Xinkeshu Tablets,Huangyangning Tablets,and Ginkgo Biloba.The main problems of unreasonable prescriptions were unclear syndrome differentiation and repeated medication.Conclusion There are many types of proprietary Chinese medicines used in the treatment of coronary heart disease in this center,and most of them are used in a standardized manner,but there are unreasonable phenomena such as lack of TCM syndrome differentiation and repeated use of medicines.The relevant knowledge training of general practitioners should be strengthened,and improve the information system,optimize the variety structure,and further strengthen rational drug use.
