早期无创机械通气在急性心力衰竭患者抢救中的应用效果

Application Effect of Early Noninvasive Mechanical Ventilation in the Rescue of Patients with Acute Heart Failure

目的:探讨早期无创机械通气在急性心力衰竭患者抢救中的应用效果。方法:选取2017年8月—2022年8月在兴宁市人民医院进行抢救的急性心力衰竭患者80例为观察对象,根据治疗方法不同分为对照组和观察组,各40例。对照组在接受常规治疗1~2 h后,症状无明显改善后给予无创机械通气治疗,观察组在治疗早期接受无创机械通气治疗。比较两组临床疗效、住院时间、憋喘症状缓解时间、生命体征及血气指标[呼吸频率(RR)、心率(HR)、动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))]。结果:治疗后,观察组治疗总有效率高于对照组,差异有统计学意义(P=0.007);观察组住院时间、憋喘症状缓解时间短于对照组,差异有统计学意义(P<0.001);治疗后,观察组PaO_(2)高于对照组,RR、HR、PaCO_(2)低于对照组,差异有统计学意义(P<0.05)。结论:急性心力衰竭患者早期行无创机械通气治疗,疗效显著,能够改善患者各项生命体征,缩短住院时间、憋喘症状缓解时间,是一种可靠、安全的抢救方式。

Objective:To explore the application effect of early noninvasive mechanical ventilation in the rescue of patients with acute heart failure.Methods:A total of 80 patients with acute heart failure who were rescued in Xingning People's Hospital from August 2017 to August 2022 were selected as observation objects,and were divided into control group and observation group according to different treatment methods,with 40 cases in each group.The control group was given noninvasive mechanical ventilation after receiving routine treatment for 1-2 hours,and the observation group was given noninvasive mechanical ventilation at the early stage of treatment.The clinical efficacy,length of hospital stay,duration of relief of asthmatic symptoms,vital signs and blood gas indexes[respiratory rate(RR),heart rate(HR),arterial partial pressure of oxygen(PaO_(2))and arterial partial pressure of carbon dioxide(PaCO_(2))]of the two groups were compared.Results:After treatment,the total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P=0.007);The duration of hospital stay and relief of asthmaticsymptoms in the observation group were shorter than those in the control group,and the difference was statistically significant(P<0.001);After treatment,PaO_(2) in observation group was higher than that in control group,RR,HR and PaCO_(2) were lower than that in control group,the difference was statistically significant(P<0.05).Conclusion:Early non-invasive mechanical ventilation therapy for patients with acute heart failure has significant curative effect,which can improve the vital signs of patients,shorten the length of hospital stay and relieve the symptoms of dyspnea,and is a reliable and safe rescue method.
