超声测定胃残余量用于指导机械通气患者肠内营养支持的效果

Effect of Ultrasound Determination of Gastric Residual Volume for Guiding Enteral Nutritional Support in Mechanically Ventilated Patients

目的:研究超声测定胃残余量(GRV)用于指导机械通气患者肠内营养的应用效果。方法:选取2021年4月—2022年12月青岛市城阳区人民医院治疗的92例机械通气患者为研究对象,简单随机化分为试验组和参照组,每组46例。参照组实施回抽胃液法测定胃残余量,试验组实施超声测定胃残余量。比较两组血清白蛋白和GRV测定操作时间、感染发生率、肠内营养并发症发生率。结果:试验组治疗后血清白蛋白高于参照组,操作时间短于参照组,差异有统计学意义(P<0.001);试验组肠内营养并发症发生率低于参照组,差异有统计学意义(P=0.001);试验组感染发生率低于参照组,差异有统计学意义(P=0.009)。结论:超声测定胃残余量用于指导机械通气患者肠内营养的应用效果较好,可降低患者感染及并发症发生率,缩短测定的时间。

Objective:To study the application effect of ultrasound determination of gastric residual volume(GRV)for guiding enteral nutrition in mechanically ventilated patients.Methods:Ninety-two mechanically ventilated patients treated in Qingdao Chengyang District People's Hospital from April 2021 to December 2022 were selected as the study subjects,and they were divided into the experimental group and the reference group by simple randomization,with forty-six cases in each group.In the reference group,the gastric residual volume was determined by the method of gastric retraction,and in the experimental group,the gastric residual volume was determined by ultrasound.The operation time,incidence of infection,and complication rate of enteral nutrition of serum albumin and GRV were compared between the two groups.Results:The serum albumin of the experimental group was higher than that of the reference group after treatment,and the operation time was shorter than that of the reference group,and the difference was statistically significant(P<0.001);The complication rate of enteral nutrition in the experimental group was lower than that in the reference group,and the difference was statistically significant(P=0.001);The incidence of infection in the experimental group was lower than that in the reference group,and the difference was statistically significant(P=0.009).Conclusion:Ultrasound determination of gastric residual volume for guiding the application of enteral nutrition in mechanically ventilated patients is more effective,can reduce the incidence of infection and complications in patients,and shorten the time of determination.
