Research Progress on Factors and Improvement Measures of Drug Dispensing Errors in Hospitals

药品调剂差错是指在配药、标签、剂量等环节出现错误,导致患者接受不正确的药物治疗。医院药品调剂差错的发生可能对患者的健康和安全造成严重影响。这种差错可能由多种原因引起,如人为因素和系统因素等。人为因素包括药师的疏忽、不恰当的沟通和培训不足,而系统因素则涉及药物配药流程、信息技术系统等方面。优化药品调剂流程、强调患者安全文化、提供充分的培训和教育、引入自动化和信息技术系统支持等为当下国内大多数医院的主要应对举措。然而既往研究表明,药品调剂差错的发生是一个复杂的多因素问题。因此,本文对医院药品调剂差错因素以及改进措施的研究进展进行综述。

Drug dispensing errors refer to errors in dispensing,labeling,dosage,etc.,resulting in patients receiving incorrect drug treatment.The occurrence of drug dispensing errors in hospitals may have a serious impact on the health and safety of patients.This error may be caused by a variety of reasons,such as human factors and system factors.Human factors include negligence of pharmacists,inappropriate communication and inadequate training,while system factors involve aspects such as drug dispensing processes and information technology systems.Optimizing drug dispensing process,emphasizing patient safety culture,providing adequate training and education,and introducing automation and information technology system support are the main responses of most hospitals in China.However,previous studies have shown that the occurrence of drug dispensing errors is a complex multi-factor problem.Therefore,this paper reviewed the research progress on the factors of drug dispensing errors in hospitals and the improvement measures.
