超声引导下颈丛臂丛阻滞麻醉在锁骨骨折手术中应用效果

Application of ultrasound-guided cervical plexus brachial plexus block anesthesia in clavicle fracture surgery

目的探讨锁骨骨折内固定手术治疗患者采取超声引导下颈丛臂丛阻滞麻醉的效果。方法选取2020年3月-2022年5月本院实施锁骨骨折内固定手术治疗患者66例进行研究,用随机表将患者分为对照组(超声引导下臂丛神经阻滞麻醉,33例)和实验组(超声引导下颈丛臂丛阻滞麻醉,33例),比较2组患者麻醉效果、疼痛程度、术中血流动力学变化及麻醉相关不良反应发生率。结果实验组麻醉有效率显著高于对照组(P<0.05)。对照组术中疼痛程度显著高于实验组(P<0.05)。实验组术中HR、SpO_(2)、NIBP水平无明显变化,且各指标均优于对照组(P<0.05)。2组麻醉相关不良反应发生率无明显差异(P>0.05)。结论锁骨骨折内固定手术治疗患者应用颈丛臂丛神经阻滞麻醉,可提高患者麻醉效果,减轻患者术中疼痛程度及有效减少对患者术中血流动力学的影响,且安全性能较高。

Objective To investigate the effect of cervical plexus brachial block anesthesia in patients with clavicular fracture treated by internal fixation.Methods From March 2020 to may 2022,66 patients with clavicle fracture treated by internal fixation in our hospital were selected and randomly divided into control group(brachial plexus block anesthesia,33 cases)and experimental group(cervical plexus brachial plexus block anesthesia,33 cases).The anesthetic effect,pain degree,intraoperative hemodynamic changes and incidence of anesthesia related adverse reactions of the two groups were compared.Results he anesthesia effective rate of the experimental group was significantly higher than that of the control group(P<0.05).The degree of pain in the control group was significantly higher than that in the experimental group(P<0.05).The levels of HR,SpO_(2)and NIBP had no significant changes in the experimental group,and the indexes were better than those in the control group(P<0.05).There was no significant difference in the incidence of anesthesia related adverse reactions between the two groups(P>0.05).Conclusion the application of cervical plexus brachial plexus block anesthesia in the treatment of clavicle fracture internal fixation can improve the anesthesia effect,reduce the degree of pain and the influence on hemodynamics,and has high safety.
