无托槽隐形矫治技术中的颞下颌关节考量

摘要 无托槽隐形矫治技术具有美观性和疗效可预测性,近年来在正畸治疗中被广泛应用。然而,在应用无托槽隐形矫治软件设计过程中,因不能对髁突铰链轴的运动进行模拟,故在后牙完成三维移动后,无法对上下颌牙列与颌骨的变化情况进行预测,同样无法实现对口颌系统功能变化状态的预测。此外,在无托槽隐形矫治过程中对颞下颌关节的影响也尚无定论。文章通过结合国内外部分学者的研究结果和笔者的临床经验,阐述在无托槽隐形矫治技术中对颞下颌关节因素的考量,以期为临床医生提供参考和借鉴。 Clear aligner therapy has been widely used in orthodontic treatment in recent years due to its aesthetic features and predictable curative effect.However,in the software design process of clear aligner,because clear aligner cannot simulate the movement of condyle hinge axis,it is impossible to predict the changes of mandibular dentition and jaw after the three-dimensional movement of posterior teeth.Similarly,it is not possible to predict the functional changes of the oral and maxillary system.In addition,the influence of clear aligner on the temporomandibular joint in the treatment process is still uncertain.Based on the research results of some scholars at home and abroad and the author′s clinical experience,this paper describes the factors of temporomandibular joint in clear aligner therapy,aiming to provide reference for clinicians.
