miR-518e在鼻咽癌中的表达及意义

The expression and clinicopathological significance of miR-518e in nasopharyngeal carcinoma

摘要 目的检测鼻咽癌组织微小核糖核酸-518e(miR-518e)的表达情况,分析miR-518e与鼻咽癌转移及临床预后的关系。方法用实时荧光定量RT-PCR方法检测15例对照组鼻咽黏膜和60例鼻咽癌组织的miR-518e表达水平,分析miR-518e与鼻咽癌临床病理参数的关系;ROC曲线分析miR-518e对鼻咽癌诊断的预测价值,采用Kaplan-Meier生存曲线分析miR-518e对患者5年生存率的影响。结果miR-518e在鼻咽黏膜和鼻咽癌组织相对表达量分别为1.14(0.33,3.70)、29.95(11.51,65.66),鼻咽癌miR-518e表达水平较鼻咽黏膜组织显著上调(Z=-5.29,P=0.000);miR-518e表达强度与鼻咽癌颈部淋巴结转移和远处转移密切相关(χ^(2)=5.96,P=0.015;χ^(2)=6.79,P=0.009),与患者性别、年龄、T分期、临床分期均无显著相关性;miR-518e对鼻咽癌的受试者工作特征(ROC)曲线下面积为0.941(95%CI:0.885~0.997,P=0.000),表明miR-518e对鼻咽癌具有一定诊断效能;生存曲线分析证实,miR-518e低表达组患者5年生存率显著高于高表达组(χ^(2)=4.26,P=0.039);结论miR-518e在鼻咽癌组织中异常表达上调,可作为预测鼻咽癌转移和临床预后的分子标记物。 Objective To investigate the expression of miR-518e in nasopharyngeal carcinoma(NPC)and analyze the correlation between miR-518e and metastasis and clinical prognosis of NPC.Methods Real-time fluorescence quantitative RT-PCR was used to detect the expression levels of miR-518e in 15 control group nasopharyngeal mucosa samples and 60 nasopharyngeal carcinoma tissue samples.The relationship between miR-518e and clinical pathological parameters of nasopharyngeal carcinoma was analyzed.ROC curve analysis was performed to evaluate the predictive value of miR-518e for diagnosing nasopharyngeal carcinoma,and Kaplan-Meier survival curve analysis was used to evaluate the impact of miR-518e on the 5-year survival rate of patients.Results The relative expression levels of miR-518e in nasopharyngeal mucosa and nasopharyngeal carcinoma tissue were 1.14(0.33,3.70)and 29.95(11.51,65.66),respectively.The expression level of miR-518e in nasopharyngeal carcinoma tissue was significantly up-regulated compared with nasopharyngeal mucosa tissue(Z=-5.29,P=0.000).The expression intensity of miR-518e was closely correlated with distant metastasis and cervical lymph node metastasis of nasopharyngeal carcinoma(χ^(2)=5.96,P=0.015;χ^(2)=6.79,P=0.009),but had no significant correlation with patient gender,age,T staging,and clinical staging.The ROC curve analysis showed that the area under the curve of miR-518e for diagnosing nasopharyngeal carcinoma was 0.941(95%CI:0.885-0.997,P=0.000),indicating that miR-518e has a certain diagnostic efficacy for nasopharyngeal carcinoma.The survival curve analysis confirmed that the 5-year survival rate of patients with low expression of miR-518e was significantly higher than that of patients with high expression(χ^(2)=4.26,P=0.039).Conclusion The expressions of miR-518e is aberrantly up-regulated in NPC,and can be used as a molecular marker to predict metastasis and prognosis of NPC.
