应对方式在冠心病患者疾病感知与生活质量间的中介效应

Mediating effect of coping style between illness perception and quality of life in patients with coronary heart disease

目的:探讨应对方式对冠心病(CHD)患者疾病感知与生活质量的影响并建立结构方程模型。方法:选取2022年3~8月弋矶山医院CHD患者297例,使用疾病感知问卷(B-IPQ)、医学应对方式问卷(MCMQ)、西雅图心绞痛量表(SAQ)进行横断面调查。结果:CHD患者SAQ得分(52.56±19.14)分;患者生活质量与疾病感知、回避及屈服应对方式均呈负相关(r=-0.318、-0.475、-0.369,P<0.01),与面对应对方式呈正相关(r=0.395,P<0.01);面对、回避及屈服应对方式在疾病感知和生活质量间起部分中介作用(β=-0.074、-0.137、-0.059,P<0.001),中介效应共占总效应的62.93%。结论:CHD患者生活质量处于中等偏低水平,制定有效的疾病认知干预措施,提高患者疾病正向应对能力,可以改善患者的身心健康状况。

Objective:To investigate the effect of coping style on disease perception and quality of life in patients with coronary heart disease(CHD),and establish a structural equation model.Methods:297 CHD patients were recruited from the First Affiliated Hospital of Wannan Medical College between March and August of 2022,and underwent cross-sectional investigation using Brief Illness Perception Questionnaire(B-IPQ),Medical Coping Modes Questionnaire(MCMQ)and Seattle Angina Questionnaire(SAQ)scale.Results:The SAQ score was 52.56±19.14 for CHD patients,and their quality of life was negatively correlated with illness perception,avoidance and submission coping style(r=-0.318,-0.475 and-0.369,respectively,P<0.01),yet positively correlated with facing coping style(r=0.395,P<0.01).There were mediating effects between illness perception,avoidance and surrender coping styles(β=-0.074,-0.137 and-0.059,P<0.001),accounting for 62.93%of the total effect.Conclusion:The quality of life of CHD patients is at a moderate and low level.Effective disease cognition intervention measures should be developed to improve the positive coping ability of such patients so as to boost their physical and mental health status.
