槐杞黄颗粒联合泼尼松治疗过敏性紫癜急性期患儿效果及对肾损害、免疫功能影响研究

Effect of Huaiqihuang Granule combined with prednisone on children with henoch-schonlein purpura at acute stage and its influence on renal damage and immune function

目的探讨槐杞黄颗粒联合泼尼松治疗急性期过敏性紫癜(HSP)患儿的效果及对肾功能、免疫功能的影响。方法选取自2018年1月至2022年12月收治的103例HSP急性期患儿为研究对象,按治疗方式分为泼尼松组泼尼松(n=51)与联合组(n=52)。泼尼松组给予泼尼松治疗,联合组在泼尼松基础上联合槐杞黄颗粒治疗。比较两组治疗效果、肾功能指标(24 h尿蛋白定量、尿微量白蛋白)、免疫功能指标(CD4+、CD8+、CD4+/CD8+)及不良反应发生率。结果联合组治疗有效率高于泼尼松组,差异有统计学意义(P<0.05)。治疗前,两组肾功能指标比较,差异无统计学意义(P>0.05);治疗后,两组24 h尿蛋白定量、尿微量白蛋白水平均下降,且联合组水平低于泼尼松组,差异有统计学意义(P<0.05)。治疗前,两组免疫指标比较,差异无统计学意义(P>0.05);治疗后,两组CD4+、CD4+/CD8+上升,CD8+下降,且联合组水平优于泼尼松组,差异有统计学意义(P<0.05)。结论槐杞黄颗粒联合泼尼松治疗能有效改善急性期HSP患儿症状,减轻肾损害,提高机体免疫力,且安全有效。

Objective To investigate the effect of Huaiqihuang Granule combined with prednisone in the treatment of children withacute henoch-schonlein purpura(HSP)and its influence on renal function and immune function.Methods A total of 103 children with acute HSP treated from January 2018 to December 2022 were selected and divided into prednisone group(n=51)and combined group(n=52)according to treatment methods.The prednisone group was treated with prednisone,and the combined group was treated with Huaiqihuang granules on the basis of prednisone.The therapeutic effects of the two groups,renal function index(24 h urine protein quantity,urine microalbumin),immune function index(CD4+,CD8+,CD4+/CD8+),and the incidence of adverse reactions were compared.Results The treatment effective rate of combination group was higher than that of prednisone group,and the difference was statistically significant(P<0.05).Before the treatment,there was no significant difference in renal function indexes between the two groups(P>0.05).After the treatment,24 h urinary protein quantitative and urinary microalbuminlevels were decreased in both groups,and the combined group was lower than prednisone group,with statistical significance(P<0.05).Before the treatment,there was no significant difference in immune indexes between the two groups(P>0.05).After the treatment,CD4+,CD4+/CD8+increased and CD8+decreased in the two groups,and the level of combined group was better than that of prednisone group,the difference was statistically significant(P<0.05).Conclusion Huaiqihuang Granule combined with prednisone can effectively improve the symptoms of acute HSP children,reduce kidney damage,improve immunity,and is safe and effective.
