Application Value of HPV E6/E7 mRNA in Early Screening of Cervical Lesions

目的探究HPV E6/E7 mRNA检测在宫颈病变早期筛查中的应用价值。方法选取行宫颈病变筛查的患者90例,以病理活检结果为金标准,将患者分成宫颈炎症组、CIN1组、CIN2组、CIN3组以及宫颈癌组,比较HPV E6/E7 mRNA与HPV DNA对早期宫颈病变的筛查价值,并分析HPV E6/E7 mRNA拷贝数与宫颈病变的关系。结果患者年龄>40岁时,HPV E6/E7 mRNA的阳性检出率显著高于HPV DNA阳性检出率(P<0.05)。宫颈炎症组、CIN1组与CIN2组中,HPV E6/E7 mRNA阳性检出率略低于HPV DNA阳性检出率,但两种检测方法之间的差异不显著(P>0.05)。CIN3组与宫颈癌组采用HPV E6/E7 mRNA检测的阳性检出率显著高于HPV DNA阳性检出率(P<0.05)。HPV E6/E7 mRNA检测的灵敏性与阳性预测值均显著高于HPV DNA检测(P<0.05),特异性略高于HPV DNA检测,但差异不显著(P>0.05)。另外随着患者宫颈病变程度的加深,HPV E6/E7 mRNA拷贝数显著增多(P<0.05)。结论HPV E6/E7 mRNA检测对早期高级别宫颈病变的筛查价值高于HPV DNA检测,同时其拷贝数与患者宫颈病变程度密切相关。

Objective To investigate the application value of HPV E6/E7 mRNA in early screening of cervical lesions.Methods 90 patients who underwent screening of cervical lesions were selected.With pathological results as the golden standard,the patients were divided into cervical inflammation group,CIN1 group,CIN2 group,CIN3 group,and cervical cancer group.The value of HPV E6/E7 mRNA and HPV DNA in early screening of cervical lesions was comparatively analyzed.The relationship between HPV E6/E7 mRNA copy number and cervical lesions was analyzed.Results The positive detection rate of HPV E6/E7 mRNA was significantly higher than that of HPV DNA in patients over 40 years old(P<0.05).In cervical inflammation group,CIN1 group and CIN2 group,the positive detection rate of HPV E6/E7 mRNA was slightly lower than that of HPV DNA(P>0.05).In CIN3 group and cervical cancer group,the positive detection rate of HPV E6/E7 mRNA was significantly higher than that of HPV DNA(P<0.05).The sensitivity and positive predictive value of HPV E6/E7 mRNA detection were significantly higher than those of HPV DNA detection(P<0.05),and the specificity was slightly higher than that of HPV DNA detection(P>0.05).With the aggravation of cervical lesions,HPV E6/E7 mRNA copy number increased significantly(P<0.05).Conclusion HPV E6/E7 mRNA detection is superior to HPV DNA in screnning early high-grade cervical lesions,and its copy number is closely related to the severity of cervical lesions.
