晚期Luminal型乳腺癌孕激素受体表达状况与其化疗疗效相关性研究

Correlation Between Progesterone Receptor Expression and Chemotherapy Efficacy in Advanced Luminal Breast Cancer

目的探讨晚期Luminal型乳腺癌孕激素受体(PR)表达与化疗疗效的相关性。方法选取行一线化疗的68例女性晚期Luminal型乳腺癌患者,依据PR阳性率的高低分为PR低表达组(n=35,阳性率<20%)与PR高表达组(n=33,阳性率≥20%)。全部患者均采用吉西他滨+顺铂(GP)方案化疗,21 d为1个周期,共化疗4个周期。于化疗结束后评估疗效,同时随访3年,分析PR高、低表达与晚期Luminal型乳腺癌患者生存周期的关系。结果PR高表达组的治疗总有效率[93.94%(31/33)]高于PR低表达组[74.29%(26/35)],有统计学差异(P<0.05);Kaplan-Meier结果显示:PR高表达组的3年生存率高于PR低表达组,有统计学差异(P<0.05)。结论晚期Luminal型乳腺癌患者PR表达状况与化疗疗效相关,在PR低表达的晚期Luminal型乳腺癌患者中采取GP化疗方案治疗效果较差,且生存率更低,临床需予以高度的重视,及时测定PR表达水平,据此采取针对性治疗,以增强治疗效果,尽可能改善患者预后。

Objective To investigate the relationship between the expression of progesterone receptor(PR)in advanced Luminal breast cancer and the efficacy of chemotherapy.Methods 68 female patients with advanced Luminal breast cancer who received first-line chemotherapy were divided into 2 groups according to the positive rate of PR:low PR expression group(n=35,positive rate<20%)and high PR expression group(n=33,positive rate≥20%).All patients were treated with gemcitabine plus cisplatin(GP)regimen chemotherapy,with 1 cycle of 21 days and a total of 4 cycles of chemotherapy.The efficacy was evaluated after chemotherapy and the patients were followed up for 3 years to analyze the relationship between the high and low expression of PR and the life cycle of patients with advanced Luminal breast cancer.Results The total effective rate of treatment in the high PR expression group[93.94%(31/33)]was higher than that in the low PR expression group[74.29%(26/35)],with a statistical difference(P<0.05);The Kaplan Meier results showed that the 3-year survival rate of the PR high expression group was higher than that of the PR low expression group,with a statistical difference(P<0.05).Conclusion PR expression in patients with advanced Luminal breast cancer is related to the efficacy of chemotherapy.In patients with advanced Luminal breast cancer with low PR expression,GP chemotherapy is less effective and has a lower survival rate.Clinical attention should be paid to timely determine the level of PR expression,and targeted treatment should be taken to enhance the therapeutic effect and improve the prognosis of patients as much as possible.
