球拍形切口乳房保乳成形术治疗乳腺癌的效果分析

Effect of Breast Conserving Plastic Surgery with Racket Shaped Incision in the Treatment of Breast Cancer

目的探讨球拍形切口乳房保乳成形术应用于乳腺癌治疗的效果。方法选取乳腺癌患者114例,按照随机数字表法分为试验组(n=57)和常规组(n=57),常规组使用常规保乳手术,试验组使用球拍形切口乳房保乳成形术。对比2组围术期指标、乳房美容效果、血清肿瘤标志物[组织癌胚抗原(CEA)、糖类抗原15-3(CA15-3)、多肽抗原(TPA)]、免疫能力(CD3^(+)、CD4^(+))、睡眠质量、生活水平、并发症变化。结果与常规组比较,试验组手术时长、住院时长、切口大小更短,且术中出血少(P<0.05)。与常规组比较,试验组质地与弹性、对称性、皮肤颜色、凹陷程度、乳头横向偏移距离、乳头纵向偏移距离等分值更高(P<0.05)。与常规组比较,术后6个月试验组TPA、CEA、CA15-3水平更低(P<0.05)。与常规组比较,术后3 d试验组CD3^(+)、CD4^(+)指数水平更高(P<0.05)。与常规组比较,术后6个月试验组生活水平分值更高,且睡眠质量分值更低(P<0.05)。试验组并发症7.02%(4/57),较常规组22.81%(13/57)低(P<0.05)。结论球拍形切口乳房成形术应用于乳腺癌治疗,可以改善围术期指标,提高乳房美容效果,降低血清肿瘤标志物,调节免疫能力,改善睡眠质量和生活水平,减少并发症。

Objective To investigate the effect of breast conserving plastic surgery with racket incision in the treatment of breast cancer.Methods 114 patients with breast cancer were randomly divided into test group(n=57)and routine group(n=57).Routine breast conserving surgery was used in the routine group,and breast conserving plastic surgery with racket shaped incision was used in the test group.The perioperative indexes,breast cosmetic effect,serum tumor markers[tissue carcinoembryonic antigen(CEA),carbohydrate antigen 15-3(CA15-3),polypeptide antigen(TPA)],immune ability(CD3^(+),CD4^(+)),sleep quality,living standard and complications were compared between the two groups.Results Compared with the routine group,the experimental group had shorter operation time,hospital stay,incision size and less intraoperative bleeding(P<0.05);Compared with the routine group,the scores of texture and elasticity,symmetry,skin color,depression degree,transverse offset distance of nipple and longitudinal offset distance of nipple in the experimental group were higher(P<0.05);Compared with the routine group,the values of tPA,CEA and CA15-3 in the experimental group were lower 6 months after operation(P<0.05);Compared with the routine group,the indexes of CD3^(+) and CD4^(+) in the experimental group were lower three days after operation(P<0.05);Compared with the routine group,the score of living standard in the experimental group was higher and the score of sleep quality was lower 6 months after operation(P<0.05);The complications in the experimental group were 7.02%(4/57)lower than that in the routine group(22.81%(13/57)(P<0.05).Conclusion For patients with breast cancer,racket incision mastoplasty can improve perioperative indicators,improve breast cosmetic effect,reduce serum tumor markers,regulate immune ability,improve sleep quality and living standards,and reduce complications.
