经尿道等离子体双极电切术治疗膀胱肿瘤的疗效及安全性评价

Evaluation of the Efficacy and Safety of Transurethral Plasma Bipolar Resection for Bladder Tumors

目的评价经尿道等离子体双极电切术(PKRP)治疗膀胱肿瘤的疗效及安全性。方法采用随机数字表法将94例膀胱肿瘤患者分为观察组与对照组,各47例。观察组应用PKRP治疗,对照组采用开放性膀胱切除术治疗,随访2年。比较两组手术指标、血钠及血红蛋白水平、并发症发生率、复发率。结果观察组术中出血量[(46.53±5.42)ml]小于对照组,手术时间[(56.85±5.98)min]、导尿管留置时间[(4.91±0.80)d]、术后住院时间[(6.96±1.37)d]均短于对照组,有统计学差异(P<0.05);术后12 h,两组血钠、血红蛋白水平均降低,但观察组高于对照组,有统计学差异(P<0.05);观察组术后并发症发生率(8.51%)及术后6个月、术后1年、术后2年的复发率(10.64%、19.15%、23.40%)低于对照组,有统计学差异(P<0.05)。结论PKRP治疗膀胱肿瘤疗效确切,能缩短治疗时间,减少出血量,降低疾病复发风险,且并发症少,安全性较高。

Objective To evaluate the efficacy and safety of transurethral plasma bipolar resection(PKRP)for the treatment of bladder tumors.Methods 94 patients with bladder tumors were divided into the observation group and the control group using a random number table method,with 47 cases in each group.The observation group was treated with PKRP,while the control group was treated with open cystectomy,with a follow-up of 2 years.Compare the surgical indicators,blood sodium and hemoglobin levels,incidence of complications,and recurrence rate between the 2 groups.Results The amount of intraoperative bleeding[(46.53±5.42)ml]in the observation group was less than that in the control group,and the operation time[(56.85±5.98)min],the retention time of catheter[(4.91±0.80)d],and the postoperative hospital stay[(6.96±1.37)d]were all shorter than those in the control group,with statistical differences(P<0.05);At 12 hours after surgery,the levels of blood sodium and hemoglobin in both groups decreased,but the observation group was higher than the control group,with a statistical difference(P<0.05);The incidence of postoperative complications(8.51%)and recurrence rates at 6 months,1 year,and 2 years after surgery(10.64%,19.15%,23.40%)in the observation group were lower than those in the control group,with statistical differences(P<0.05).Conclusion PKRP has a definite therapeutic effect on bladder tumors,which can shorten the treatment time,reduce bleeding,reduce the risk of disease recurrence,and have fewer complications and higher safety.
