膀胱癌与腺性膀胱炎应用增强CT扫描的临床诊断价值

Clinical Value of Contrast-enhanced CT Scan in Bladder Cancer and Cystitis Glandularis

目的探析应用增强CT扫描对膀胱癌与腺性膀胱炎患者临床诊断价值。方法纳入2018年1月至2022年1月收治的确诊为膀胱癌、腺性膀胱炎的患者,在其中随机抽取,分别为41例、37例,所有患者均接受CT平扫、CT增强扫描,分析所得图像数据对于鉴别诊断膀胱癌与腺性膀胱炎的临床诊断效能。结果膀胱癌病灶在动脉期、静脉期、延迟期、相对强化动脉期、相对强化静脉期及相对强化延迟期CT扫描的CT值高于腺性膀胱炎,差异有统计学意义(P<0.05)。动脉期相对强化CT值、静脉期相对强化CT值的AUC高于其他4个强化指标,差异有统计学意义(P<0.01),两者的AUC指标比较,差异无统计学意义(P>0.05)。对膀胱癌与腺性膀胱炎患者鉴别诊断时,动脉期相对强化、静脉期相对强化的CT阈值分别在25.85 HU、26.85 HU时特异度分别为91.89%(34/37)、86.49%(32/37),敏感度均为95.12%(39/41)。结论将增强CT应用于鉴别膀胱癌与腺性膀胱炎有一定的临床指导价值。

Objective To explore the clinical diagnostic value of contrast-enhanced CT scan in patients with bladder cancer and glandular cystitis.Methods From January 2018 to January 2022,patients diagnosed with bladder cancer and cystitis glandularis were included,and 41 and 37 cases were selected respectively.All patients received CT plain scan and CT enhanced scan,and the obtained image data were analyzed.To analyze its clinical diagnostic efficacy for differential diagnosis of bladder cancer and glandular cystitis.Results The CT values of bladder cancer lesions in the arterial phase,venous phase,delayed phase,relative enhancement arterial phase,relative enhancement venous phase and relative enhancement delayed phase were significantly higher than those in cystitis glandularis,with statistically significant differences(P<0.05).The AUC of the relative enhanced CT value in the arterial phase and the relative enhanced CT value in the venous phase was higher than the other four enhancement indexes,with statistical significance,with statistically significant differences(P<0.01),but there was no significant difference in the AUC between the two indexes(P>0.05).The CT thresholds of relative enhancement in arterial phase and relative enhancement in venous phase in the differential diagnosis of bladder cancer and glandular cystitis patients were 25.85 HU and 26.85 HU with specificity of 91.89%(34/37)and 86.49%(32/37,respectively),the sensitivity was 95.12%(39/41).Conclusion The application of contrast-enhanced CT in the identification of bladder cancer and glandular cystitis has certain clinical guiding value.
