弹性髓内钉在儿童四肢骨折中的应用

摘要 目的探讨钛制弹性髓内钉在儿童四肢骨折中的应用价值。方法选取2016年1月至2019年6月本院收治的110例四肢骨折患儿作为研究对象,按照随机数字表法分为对照组与观察组,每组55例。对照组采用切开复位钢板内固定疗法治疗,观察组采用钛制弹性髓内钉疗法治疗,比较两组围手术期指标、骨折愈合情况、临近关节活动情况、术后疼痛情况、并发症发生率。结果观察组手术时间、切口长度、住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。观察组骨折愈合率高于对照组,差异有统计学意义(P<0.05)。观察组临近关节活动优良率高于对照组,差异有统计学意义(P<0.05)。术后3、5 d时,观察组Wong-Backe评分均低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论钛制弹性髓内钉治疗四肢骨折患儿效果理想,可促进患儿术后及早恢复,减轻患儿疼痛程度,安全性较高,值得临床推广应用。 Objective To investigate the application value of titanium elastic intramedullary nail in children with limb fractures.Methods A total of 110 children with limb fractures admitted to our hospital from January 2016 to June 2019 were selected as the research subjects,and they were divided into the control group and the observation group according to the random number table method,with 55 cases in each group.The control group were treated with open reduction and internal fixation therapy,and the observation group were treated with titanium elastic intramedullary nail therapy,the perioperative indexes,fracture healing,adjacent joint activity,postoperative pain,and incidence of complication were compared between the two groups.Results The operation time,incision length and hospitalization time in the observation group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group,and the differences were statistically significant(P<0.05).The fracture healing rate in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).The excellent and good rate of adjacent joint activity in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).At 3 and 5 d after surgery,the Wong-Backe score in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Titanium elastic intramedullary nail in the treatment of children with limb fracture is ideal,which can promote early postoperative recovery of children,reduce the degree of pain,and have high safety,which is worthy of clinical promotion and application.
