血清C肽与糖化血红蛋白在糖尿病诊断中的应用价值

摘要 目的探究血清C肽(C-PR)与糖化血红蛋白(HbA1c)在糖尿病诊断中的应用价值。方法选取2018年12月至2020年12月于本院就诊的100例糖尿病患者作为观察组,另选取同期于本院体检的100名健康者作为对照组,两组均行C-PR、HbA1c、空腹血糖(FBG)餐后2 h血糖(2 h PBG)水平检测,比较两组C-PR、HbA1c、FBG、2 h PBG水平及观察组不同HbA1c水平患者C-PR、FBG、2 h PBG水平、观察组不同CPR水平和不同HbA1c水平患者并发症发生率。结果观察组HbA1c、FBG、2 h PBG水平均明显高于对照组,而C-PR水平明显低于对照组,差异有统计学意义(P<0.05)。观察组HbA1c>10%患者C-PR水平明显低于HbA1c≤10%患者,FBG及2 h PBG水平均明显高于HbA1c≤10%患者,差异有统计学意义(P<0.05)。观察组低C-PR水平患者并发症发生率明显高于C-PR水平正常患者,差异有统计学意义(P<0.05)。观察组HbA1c水平>7.5%患者神经病变、糖尿病肾病(DN)和冠心病发生率明显高于HbA1c水平≤7.5%患者,差异有统计学意义(P<0.05)。结论C-PR与HbA1c可有效反映患者的胰岛素分泌及血糖控制情况,且可一定程度上评估患者合并症发生的风险,在糖尿病患者的诊断中具有较高的应用价值。 Objective To explore the application value of serum C-peptide(C-PR)and glycosylated hemoglobin(HbA1c)in the diagnosis of diabetes mellitus.Methods 100 patients with diabetes mellitus treated in our hospital from December 2018 to December 2020 were selected as the observation group,and 100 healthy people who underwent physical examination in our hospital during the same period were selected as the control group,the levels of C-PR,HbA1c,fasting blood glucose(FBG)and 2 h postprandial blood glucose(2 h PBG)were detected in the two groups,the levels of C-PR,HbA1c,FBG,2 h PBG were compared between the two groups and the levels of C-PR,FBG,2 h PBG in patients with different HbA1c levels,as well as the incidence of complications in patients with different C-PR levels and different HbA1c levels were compared in the observation group.Results The levels of HbA1c,FBG,and 2 h PBG in the observation group were significantly higher than those in the control group,while the level of C-PR was significantly lower than that in the control group,and the differences were statistically significant(P<0.05).The C-PR level of patients with HbA1c>10%was significantly lower than that of patients with HbA1c≤10%,and the FBG and 2 h PBG levels were significantly higher than those of patients with HbA1c≤10%in the observation group,and the differences were statistically significant(P<0.05).The incidence of complications in patients with low C-PR levels was significantly higher than that in patients with normal C-PR levels in the observation group,and the difference was statistically significant(P<0.05).The incidence of neuropathy,diabetes nephropathy(DN),and coronary heart disease in patients with HbA1c levels>7.5%was significantly higher than that in patients with HbA1c levels≤7.5%in the observation group,and the difference was statistically significant(P<0.05).Conclusion C-PR and HbA1c can effectively reflect the insulin secretion and blood sugar control of patients,and can evaluate the risk of complications to certain extent,which has high application value in the diagnosis of patients with diabetes mellitus.
