掌背穿支皮瓣滑移术与掌背侧三角形皮瓣成形术在先天性并指畸形指蹼重建中的应用效果

Application effect of dorsal palmar perforator flap slippage and dorsal palmar triangular flap plasty in the reconstruction of web of congenital syndactyly

目的探讨掌背穿支皮瓣滑移术与掌背侧三角形皮瓣成形术在先天性并指畸形指蹼重建中的应用效果。方法选取2020年1月至2021年12月本院收治的100例先天性并指畸形患儿作为研究对象,采用随机抽签法分为观察组与对照组,每组50例。对照组采用掌背侧三角形皮瓣成形术,观察组采用掌背穿支皮瓣滑移术,比较两组手术疗效、术后手部功能、术后并发症发生率、家长满意度。结果观察组治疗优良率为94.00%,高于对照组70.00%,差异有统计学意义(P<0.05);观察组术后外观评分高于对照组,差异有统计学意义(P<0.05);两组运动、感觉评分比较差异无统计学意义。观察组术后并发症发生率为6.00%,低于对照组的22.00%,差异有统计学意义(P<0.05)。观察组家长总满意度为90.00%高于对照组的74.00%,差异有统计学意义(P<0.05)。结论先天性并指畸形指蹼重建中应用掌背穿支皮瓣滑移术能满足患儿手部美观性的需求,减少术后并发症,提高患儿家长满意度,值得临床推广应用。

Objective To explore the application effects of dorsal palmar perforator flap slippage and dorsal palmar triangular flap in the reconstruction of web of congenital syndactyly.Methods 100 children with congenital syndactyly admitted to our hospital from January 2020 to December 2021 were selected as the research subjects,they were divided into the observation group and the control group by random drawing method,with 50 cases in each group.The control group underwent dorsal palmar triangular flap plasty,while the observation group underwent dorsal palmar perforator flap slippage surgery,the surgical efficacy,postoperative hand function,incidence of postoperative complications,and parental satisfaction rates were compared between the two groups.Results The treatment excellent and good rate in the observation group was 94.00%,which was higher than 70.00%in the control group,the difference was statistically significant(P<0.05).The postoperative appearance score in the observation group were higher than that in the control group,and the difference was statistically significant(P<0.05),while there was no statistically significant difference in movement and sensory scores between the two groups.The incidence of postoperative complication in observation group was 6.00%,which was lower than 22.00%in the control group,the difference was statistically significant(P<0.05).The total satisfaction of parents in observation group was 90.00%,which was higher than 74.00%in control group,and the difference was statistically significant(P<0.05).Conclusion The application of dorsal palmar perforator flap slip in the reconstruction of finger web for congenital syndactyly can meet the needs of hand aesthetics of children,reduce postoperative complications,and improve the satisfaction of parents of children,which is worthy of clinical promotion and application.
