外周血Th22与IL-22联合多层螺旋CT在儿童重症MPP早期诊断中的应用价值

Application value of peripheral blood Th22 and IL-22 combined with multi-slice spiral CT in early diagnosis of severe MPP in children

目的探讨外周血辅助性T细胞(Th)22、血清白细胞介素-22(IL-22)联合多层螺旋CT在儿童重症肺炎支原体肺炎(MPP)早期诊断中的应用价值。方法选取2019年7月至2020年7月本院收治的35例重症MPP患儿作为观察组,另选取同期35名健康体检儿童作为对照组。两组均予以外周血指标检测和多层螺旋CT检查,分析重症MPP患儿多层螺旋CT影像学特征,比较两组外周血Th22和血清IL-22、25-羟基维生素D_(3)[25-(OH)D_(3)]水平。结果重症MPP患儿多层螺旋CT影像学显示,病变部位以左肺、右肺、双肺为主;病变类型包括肺叶/段实变、小叶性肺炎、支气管炎3类。观察组外周血Th22、血清IL-22水平均高于对照组,血清25-(OH)D_(3)水平低于对照组,差异有统计学意义(P<0.05)。结论外周血Th22与血清IL-22联合多层螺旋CT在重症MPP患儿早期诊断中发挥重要作用,可为重症MPP患儿早期诊治提供有价值的参考依据,值得临床推广应用。

Objective To explore the application value of peripheral blood helper T cell(Th)22,serum interleukin-22(IL-22)combined with multi-slice spiral CT in the early diagnosis of severe Mycoplasma pneumoniae pneumonia(MPP)in children.Methods 35 children with severe MPP admitted to our hospital from July 2019 to July 2020 were selected as the observation group,and 35 healthy children who underwent physical examination during the same period were selected as the control group.Both groups underwent peripheral blood indexes and multi-slice spiral CT examination,the imaging features of multi-slice spiral CT were analyzed,the levels of Th22 and IL-22 and 25-hydroxyvitamin D_(3)(25-[OH]D_(3))in peripheral blood were compared between two groups.Results Multi-slice spiral CT imaging of severe MPP children showed that the lesion site were mainly in the left lung,right lung,and bilateral lungs;the types of lesions include lung lobe/segment consolidation,lobular pneumonia,and bronchitis.The levels of peripheral blood Th22 and serum IL-22 in the observation group were higher than those in the control group,while the levels of serum 25-(OH)D_(3) were lower than that in the control group,the differences were statistically significant(P<0.05).Conclusion Peripheral blood Th22,serum IL-22 combined with multi-slice spiral CT plays an important role in the early diagnosis of severe MPP in children,which can provide valuable reference for the early diagnosis and treatment of severe MPP in children,and is worthy of clinical promotion and application.
